巨淋巴结增生症的影像学表现

CT and MRI Features of Castleman's Disease

目的:探讨巨淋巴结增生症(Castleman's disease,CD)的CT、MRI及^(18)F-FDG PET/CT表现。方法:回顾性分析经病理确诊的24例巨淋巴结增生症的影像学表现。结果:局限型CD多为透明血管型,表现为类圆形软组织结节或肿块,密度或信号较均匀,边界清晰,瘤体内见斑点状、条状、分叉状钙化;增强后肿瘤呈明显持续强化;病灶周围可见"卫星"结节及迂曲、增粗的血管影。多中心型CD多为浆细胞型,表现为多发淋巴结肿大,密度均匀,增强亦明显强化,但较透明血管型强化程度低。^(18)F-FDG PET/CT检查7例,平均SUV_(max)3.63±0.89。结论:巨淋巴结增生症的CT、MRI及^(18)F-FDG PET/CT表现具有一定的特征性,有助于临床诊断。

Objective：To analyze the CT,MRI and ^18F-FDG PET/CT imaging features of Casdeman disease and enhance our knowledge.Method：Twenty four patients with lymph node biopsy-proved or surgery-proved Casdeman disease were retrospectively reviewed in this study.Results：Eighteen patients with localized Castleman disease had hyaline vascular type and showed wellcircumscribed mass with soft-tissue density,punctate,linear or bifurcate calcification on non-enhanced CT imaging.Density or the signal was uniform with significant enhancement.Enhancing vessels and ＂satellite＂ nodes were demonstrated around localized masses.Multicentric CD＇s masses of plasma cell type with homogeneous density also showed enhancement after injection of contrast media but appeared to reveal a less increase than the hyaline vascular type.Conclusion：The CT,MRI and ^18F-FDG PET/CT manifestations of Casdeman＇s disease have certain characteristics which is helpful for clinical diagnosis.
