针刀、手法、三氧松解术治疗腰椎间盘突出症80例临床观察

Clinical observation on treating 80 cases of lumbar disc herniation by acupuncture

目的:观察针刀结合剪力运动手法及局部注射三氧治疗L4/5、L5/S1椎间盘后外侧突出的临床疗效并探讨其机制。方法:将120例患者按照就诊顺序进行编号,并通过DPS处理系统按照2∶1比例进行随机分组,针刀治疗组80例、针灸治疗组40例。治疗组(针刀治疗组)进行针刀结合剪力运动手法及三氧松解术,对照组(针灸治疗组)行常规针刺治疗,观察患者治疗后临床症状缓解等情况,判定疗效,并随访1年,观察复发率。结果:疗程结束后,治疗组痊愈率为76.00%,对照组痊愈率为45.00%,两组比较差异具有统计学意义(P<0.01)。结论:针刀、剪力运动、三氧注射,可通过快速直腿抬高,松解神经根粘连,改变突出物与神经根解剖关系而产生治疗效应,其治疗效果明显优于针刺治疗。

Objective：To observe clinical efficacy of knife acupuncture on L4/5, L5/S1 intervertebral discs and its mechanism. Methods： 120 patients, by DPS processing system according to the ratio of 2∶1, were randomly divided into two groups, the knife acupuncture treatment group （80 cases） and the acupuncture treatment group （40 cases）. The treatment group recieved acupotomology plus manipulation and three oxygen neurolysis;the control group recieved routine acupuncture therapy, followed up for 1 year. Results：After the treatment, the efifcacy was 76%in the treatment group and 45%in the control group;the difference was statistically signiifcant （P〈0.01）. Conclusion：Knife acupuncture and manipulation, three oxygen injection were obviously better than acupuncture in the treatment.
