肌肉活检病理检查在小儿神经肌肉疾病中的应用 被引量:3

摘要 目的:探讨肌肉活检病理检查在小儿神经肌肉疾病中的应用价值。方法:回顾性分析本院儿童神经内分泌专科2010年1月至2014年12月患儿肌肉活检和骨骼肌病理学特点。开放性肌肉活检法取出骨骼肌标本经低温干冰浴法(丙酮)冷冻固定处理后,8μm冰冻连续切片行组织化学、酶组织化学和免疫组织化学染色等检测分析。结果:36例患儿进行了肌肉活检,肌肉病理诊断统计显示肌源性损害者31例,神经源性损害者4例,不能明确诊断者1例。在肌肉病理特征基础上进一步行相关基因检查,35例(97.22%)患儿得到明确临床诊断。其中肌源性肌损害包括Duchenne型肌营养不良20例,Becker型肌营养不良2例,感染性肌炎2例,皮肌炎2例,多发性肌炎1例,肢带型肌营养不良2C型1例,先天性肌营养不良1例,先天性肌病1例,肉碱棕榈酰基转移酶缺乏引起脂质沉积性肌病1例。神经源性损害包括脊髓性肌萎缩症2例,癫痫性脑病2例。结论:开展肌肉活检病理检查是明确小儿神经肌肉疾病的诊断和鉴别诊断重要手段,为小儿神经肌肉疾病临床治疗、康复和预后提供关键线索。
