超声乳化及前段玻切治疗睫状王不阻塞性青光眼

Treatment of ciliary block glaucoma with phacoemulsification and anterior vitrectomy

目的评价晶状体超声乳化人工晶状体植入联合前段玻璃体切除治疗睫状环阻塞性青光眼的效果。方法15例（15眼）发生于小梁切除术后的睫状环阻塞性青光眼接受手术治疗。术中先切除前段玻璃体至眼压降低，再行透明角膜切口晶体状超声乳化吸出，再次前段玻璃体切除，包括玻璃体前界膜及品状体后囊切开，使眼压进一步稳定。囊袋内植入人工晶状体。结果所有患者术后视力得到提高，前房形成良好，眼压控制稳定。结论晶状体超声乳化人工晶状体植入联合前段玻璃体切除治疗睫状环阻塞性青光眼安全有效，术中行玻璃体前界膜及晶状体后囊的切开更为重要。

Objective To investigate the efficacy of phaeoemulsifieation combined with intraoeular lens implantation and anterior vitrectomy for the treatment of ciliary block glaucoma. Methods Fifteen eyes of 15 patients with ciliary block glaucoma after trabeculeetomy were enrolled. In the operation, the anterior vitrectomy was performed first to reduce the intraocular pressure. Then phacoemulsification was performed and anterior vitreous body was vitrectomized again including the incision of anterior hyaloid membrane and posterior capsule. Finally, the IOL was inserted in the capsular bag. Results All patients were cured with normal intraocular pressure, normal anterior chamber depth and the visual acuity improved. Conclusion The treatment of ciliary block glaucoma by phaco ＋ IOL, combined with anterior vitrectomy is safe and efficient and the incision of anterior hyaloid membrane and posterior capsule during the surgery is also important.
