角膜错位切开及层间分离治疗大泡性角膜病变

Vicarious incision on lamellar cornea combined with laminal separation covering for bullous keratopathy

目的探讨一种适用于基层医院治疗大泡性角膜病变的新方法。方法对不具备行角膜移植和角膜内皮移植条件的大泡性角膜病变行角膜板层错位切开联合层间分离及羊膜覆盖术，观察其疗效。结果本组8例（8眼）手术后，眼痛、流泪等眼部刺激症状消失，角膜上皮修复，有效缓解了症状，保留了眼球。结论角膜板层错位切开联合层间分离及羊膜覆盖术对于不具备行角膜移植和角膜内皮移植条件的大泡性角膜病变患者可有效缓解症状，减轻痛苦，改善患者生活质量，且操作简单，并发症少。

Objective To study a new therapy for bullous keratopathy in primary hospitals. Methods Vicarious incision of lamellar cornea combined with luminal separation and amniotic membrane covering was conducted on the patients with bullous keratopathy who could not be clone keratoplasty or endothelium keratoplasty, and the curative effects were observed. Results After the operation, ophthalmodynia, lachrymation and other ocular irritation symptoms were disappeared in 8 eyes of 8 cases. The corneal epithelium was repaired and symptoms were relieved effectively. The eyeball was kept. Conclusion Vicarious incision of lamellar cornea combined with luminal separation and amniotic membrane covering which is easy and has less complications can relieve the pain, effectively relieve the symptoms, and improve the life quality of patients with bullous keratopathy who can not be done keratoplasty or endothelium keratoplasty.
