WADiana全自动配血系统在预输血患者抗体筛查中的应用

Research on applied value of WADiana automated blood analysis system in screening of irregular antibody in pre transfusion patients

目的:探讨WADiana全自动配血系统在患者不规则抗体筛查中的应用价值,了解本地区预输血患者不规则抗体检出率、分布特点以及对预输血患者进行抗体筛查的意义。方法:采用微柱凝胶法对9842例预输血患者进行不规则抗体筛查,筛查阳性者用谱细胞进一步做特异性鉴定。结果:19842例预输血患者检出不规则抗体79例,阳性检出率为0.80%,其中以Rh血型系统和MNSs血型系统多见,分别占41.77%和15.19%。非特异性抗体占25.32%,IgG类占68.35%;2女性患者检出率为1.10%,男性患者检出率为0.54%,女性高于男性,差异有统计学意义(x^2=9.49,P<0.01)。有输血和(或)妊娠史的患者检出率为1.38%,高于无输血和(或)妊娠史患者的检出率,差异有统计学意义(x^2=43.82,P<0.01)。结论:预输血患者不规则抗体检出率较高,在输血前对其进行抗体筛查有助于血液选择,对于保证输血安全具有重要意义。

Objective： To discuss the application value of WADiana automated blood analysis system in irregular antibody screening, and understand the detection rate and distribution characteristics of irregular antibody among pre transfusion patients in our region and explore the meaning of antibody screening for pre transfusion patients. Methods： Nine thousand eight hundred and forty two cases of pre transfusion patients in our hospital were carried irregular antibody screening by using the micro column gel method, the positive specimens was made further identification by using specific spectrum cells. Results：（1）79 cases of irregular antibodies were detected in 9842 patients with pre transfusion and positive detection rate of 0.803%, among them Rh system and MNSs system were account for 41.77%（33/79） and 15.19%（12/79） respectively, nonspecific antibodies account for 25.32%（20/79）, most of of irregular antibodies were Ig G class, accounted for 68.35%（54/79）.（2）The detection rate was 1.102%（50/4536） in female, and 0.547%（29/5306） in male, that in female were higher（x^2=9.49, P〈0.01）. The detection rate in patients with a history of blood transfusion/pregnancy was 1.389%（67/4822）, and higher than the rate in patients with no history of blood transfusion/pregnancy（x^2=43.82, P〈0.01）. Conclusion： The detection rate of irregular antibody among pre transfusion patients in our region is higher, the antibody screening before blood transfusion would help the blood to choose, and is of great significance to ensure the safety of blood transfusion.
