人工流产创伤性应激障碍及其相关因素分析

Analysis of Posttramatic stress disorder and the related factor after abortion

的：采用创伤后应激障碍筛查表研究人工流产患者创伤性应激障碍的发病率及相关因素。方法：2009年9月~2010年7月门诊要求行人工流产术的孕妇200例作为调查对象，妊娠周数﹤12周，分别完成一般情况调查表，症状自评量表，采用 Logistic 回归分析创伤性应激障碍阳性患者的危险因素。用χ2检验检测创伤后应激障碍阳性患者与阴性患者的一般情况及症状自评量表的差别。结果：200例患者全部完成调查问卷，经创伤性应激障碍筛查表诊断阳性患者86例，占人工流产人数的4.3％；创伤后应激障碍阳性患者的症状自评量表抑郁、焦虑明显高于非患者（P ﹤0.05）；创伤后应激障碍阳性患者与非患者在人工流产次数，人工流产并发症（感染、不孕、月经不调）各项之间有统计学意义（P ﹤0.05），在人工流产方式、婚姻、职业、收入等各方面无统计学意义（P ﹥0.05）。创伤后应激障碍发生的相关因素是焦虑、抑郁、人工流产后并发症（P ﹤0.05）。结论：人工流产者普遍存在抑郁、焦虑，创伤后应激障碍发病率高，与创伤性应激障碍密切相关的是焦虑、抑郁、人工流产后并发症。

Objective To probe into the incidence rate of post - traumatic stress disorder（PTSD）after abortion and related factor by use of（PTSD）table. Method 200 cases of outpatient women requirement for abortion were selected as research object. Gestational age ﹤12 weeks,200 patients,respectively,completed the general information questionnaire and SCL - 90,Logistic regression analysis of risk fac-tors of post traumatic stress disorder in patients with positive. The chi square test was used to detect the general condition and symptoms of post - traumatic stress disorder in patients with positive and negative patients with self rating scale of the difference. Results 200 patients completed the questionnaire,86 positive cases of patients were diagnosed by post traumatic stress disorder diagnosis screening table,ac-counting for 4. 3% of the number of abortion;posttraumatic stress disorder in patients with positive symptoms self rating scale of depression, anxiety and significantly higher than that of control group（P ﹤ 0. 05）;PTSD positive patients and non patients in abortion the number of times,abortion complications（infection,infertility,irregular menstruation）had statistical significance between items（P ﹤ 0. 05）,there was no statistical significance in all aspects of abortion way,marriage,occupation,income（P ﹥ 0. 05）. The related factors of PTSD was anxiety, depression,post abortion complications（P ﹤ 0. 05）. Conclusion Abortion is prevalent in patients with depression,anxiety,posttraumatic stress disorder,the disease incidence rate is high,and post traumatic stress disorder is closely related to anxiety,depression,post abortion complications.
