脑卒中患者抑郁和生活质量状况及其影响因素分析

Analysis on Influencing Factors of QOL Status and Depression in Patients with Cerebral Stroke

目的探讨脑卒中患者抑郁和生活质量（QOL）状况及其影响因素。方法采用抑郁自评量表（SDS）和汉化版脑卒中特定生活质量量表（SS-QOL）对2012年2月至2014年7月牡丹江医学院红旗医院就诊的102例脑卒中患者的抑郁状况和生活质量进行评估。结果 102例脑卒中患者有抑郁症状者45例,发生率为44.1%。Logistic回归分析结果显示,学历（OR=5.876,95%CI 1.204~17.345）、配偶（OR=8.637,95%CI 1.307~22.346）、医疗付费方式（OR=6.849,95%CI 1.690~28.304）是脑卒中患者并发抑郁症状的影响因素。脑卒中患者生活质量影响因素的多元线性回归方程：Y=3.182＋0.027X2＋0.039X3＋0.012X4＋0.029X5-0.049X6,配偶、受教育程度、月收入、付费方式和抑郁程度对患者的生活质量有影响。结论学历、配偶、医疗付费方式是脑卒中患并发抑郁的独立影响因素,配偶健在、受教育程度高、月收入高、非自费的付费方式能促进患者的生活质量改善,而抑郁程度越严重越不利于患者的生活质量改善。

Objective To investigate depression and quality of life（ QOL） and its influencing factors in patients with cerebral stroke. Methods The self-rating depression scale（ SDS） and the Chinese version of stroke specific quality of life scale（ SS-QOL） were used to evaluate depressive status and quality of life of102 patients with stroke in Hongqi Hospital of Mudanjiang Medical College from Feb. 2012 to Jul.2014. Results Among the 102 cases of stroke,45 cases had depression,the incidence was 44. 1%. The results of Logistic regression analysis showed： education[OR = 5. 876,95% CI 1. 204-17. 345 ],spouse[OR = 8. 637,95% CI 1. 307-22. 346],medical payment way[OR = 6. 849,95% CI 1. 690-28. 304] were independent risk factors of cerebral apoplexy complicated with depression; the multiple linear regression equation of influence of quality of life in stroke patients： Y = 3. 182 ＋ 0. 027X2＋ 0. 039X3＋ 0. 012X4＋0. 029X5- 0. 049X6,and spouse,education,monthly income,payment way and degree of depression had an influence on the quality of life of the patients. Conclusion Education degree,spouse,medical payment way are independent risk factors of depression in stroke patients,the living spouse,higher education level,higher monthly income,non-self-payment can help patients to improve quality of life,while the severer the depression level,the less conducive to improve the quality of life of the patients.
