妊娠期糖尿病妊娠中晚期胰岛素抵抗及胰岛B细胞功能的研究 被引量:29

摘要 目的探讨妊娠期糖尿病(GDM)孕妇妊娠中晚期胰岛素抵抗与胰岛B细胞功能变化及其与血糖正常孕妇的差别。方法以2013年3月-2014年2月在该院进行产前检查的105例孕妇为研究对象,均于妊娠20~24周进行空腹葡萄糖耐量试验(OGTT)及胰岛素释放试验检测,其中54例诊断为GDM(观察组),其余51例血糖正常(对照组)。所有研究对象在孕32~36周时再次行OGTT及胰岛素释放试验检测,对比分析不同妊娠阶段两组孕妇胰岛素抵抗及胰岛B细胞功能的变化。结果观察组与对照组从自身纵向上观察,从时间主效应上来说HOMA-β相比差异有统计学意义(F=7.835,P=0.009);从组间主效应上来说△I_(30)/△G_(30)相比差异有统计学意义(F=6.031,P=0.013),对照组孕晚期OGTT血糖血线下面积(AUCG)明显高于孕中期(F=7.213,P=0.007),而观察组孕中期AUGG与孕晚期相比差异无统计学意义(F=3.836,P=0.121);AUCG的组间主效应相比差异有统计学意义(F=6.564,P=0.002)。观察组和对照组稳态模式胰岛素抵抗指数(HOMA-IR)从孕中期到孕晚期呈上升趋势,并且观察组高于对照组;在观察组和对照组中,胰岛B细胞分泌指数(HOMA-β)随着妊娠的进展呈逐渐升高趋势;观察组早期胰岛素分泌指数(△I_(30)/△G_(30))低于对照组,△I_(30)/△G_(30)在对照组中随着妊娠的进展呈下降趋势,而观察组呈缓慢上升趋势;观察组混合胰岛素敏感度(ISIcomp)低于对照组,随着妊娠的进展,两组均呈明显下降趋势,且对照组较观察组下降趋势更明显;AUCG在观察组中明显高于对照组,观察组AUCG随着妊娠进展呈下降趋势,而对照组呈逐渐升高趋势。结论正常孕妇及GDM孕妇胰岛素抵抗程度均增加,正常孕妇胰岛素抵抗程度明显低于GDM孕妇,正常孕妇及GDM孕妇胰岛B细胞功能均增强;GDM孕妇早期胰岛素分泌功能明显低于正常孕妇。因此,GDM发生的重要机制可能是胰岛素代偿性分泌不足和胰岛素抵抗。
