3D-CTA在颅内血管畸形急诊手术中的诊断和应用研究 被引量:3

摘要 目的探讨三维CT血管造影(3D-CTA)在颅内血管畸形诊断中的应用价值。方法分析患者3DCTA资料与数字减影脑血管造影(DSA)资料及术中观察情况比较,评价3D-CTA在颅内血管畸形诊断中的可靠性。结果 3D-CTA诊断41例动静脉畸形(AVM),能清楚显示主要的供血动脉、畸形团的位置与大小及引流静脉等详细情况。但对细小的供血动脉显示不佳。8例急诊手术的患者,3D-CTA诊断结果与手术证实的结果一致。结论 3D-CTA作为一种无创的影像学检查方法,在颅内血管畸形诊断中具有重要的临床价值。尤其是对于AVM破裂出血需急诊手术、不能及时行DSA检查的患者,3D-CTA快速、准确、安全的特点,更具有突出的临床价值。 Objective To evaluate the clinical value of 3D - CTA in patients with intracranial vascular malformation. Methods 3D - CTA data of patients and DSA and observation conditions in the surgery were analyzed and compared. The reliability of 3D - CTA in diagnosis of intracranial vascular malformation was evaluated. Results 41 AVM cases were diagnosed with 3D - CTA, which were clearly showed the main blood supply arteries, the location of the abnormal group and size of drainage vein. But the small blood supply arteries were showed poorly. Eight patients with emergency surgery, the results of 3D - CTA were consistent with the results of surgery. Conclusion 3D - CTA as noninvasive imaging examination method has important clinical value in the diagnosis of intracrani- al vascular malformation, especially for patients who have AVM of bleeding rupture, need emergency surgery, cannot checked with DSA in time, and 3 D -CTA is fast, accurate and safe, and has more prominent clinical value.
