氯匹格雷联合阿司匹林治疗短暂性脑缺血发作的疗效比较

A Comparison on Effect of Clopidogrel and Aspirin on TCIA

目的：观察氯匹格雷联合阿司匹林治疗短暂性脑缺血发作的疗效,并与单用阿司匹林进行对比.方法：2012年1月~2014年9月某院收治短暂性脑缺血发作患者40例,将其随机分为两组（各20例）,实验组采用氯匹格雷联合阿司匹林方法,对照组采用阿司匹林单药方法,均持续3个月,比较两组患者的完全控制率、治疗前后血小板变化及不良反应发生率.结果：实验组短暂性脑缺血发作完全控制19例,1例进展为脑梗死,完全控制率为95.00%;对照组短暂性脑缺血发作完全控制12例,8例进展为脑梗死,完全控制率为60.00%,P 〈0.01,两组有显著性差异.实验组治疗前血小板为（168.95±43.19）,治疗后为（168.47±41.57）;对照组治疗前血小板为（197.91±65.83）,治疗后为（195.71±56.38）.两组治疗前后血小板变化均无明显差异,P 〉0.05,无统计学意义.实验组和对照组均未见颅内出血、消化道、泌尿系及皮肤黏膜出血,无过敏反应.结论：氯匹格雷联合阿司匹林治疗短暂性脑缺血更有效,且安全无毒副作用,是治疗短暂性脑缺血的理想用药方式.

Objective:To observe the effect of clopidogrel and aspirin on transient cerebral ischemic attacks and compare its effect with that of aspirin alone.Methods:During the period from January 2012 to September 2014,40 patients attacked by transient cerebral ischemia were received in some hospital,randomly divide them into 2groups,20 cases for each.The experiment group were treated with clopidogrel and aspirin while the control group adopted aspirin alone.After 3 months of treatment,compare the complete control rate,changes in blood platelet and the occurrence of adverse reaction in the 2groups.Make statistical analysis according to software SPSSll.5and t test on measurement data.Results:In experiment group,19 cases of TCIA were completely controlled,1case developed into cerebral infarction,thus the complete control rate was 95.00%;in control group,12 cases of TCIA were completely controlled,8cases developed into cerebral infarction,thus the complete control rate was 60.00%,P<0.01.There were obvious differences between the 2groups.and the content of blood platelet changed from 168.95±43.19 before treatment to 168.47±41.57 after treatment;while in control group,the content of blood platelet changed from 197.91±65.83 before treatment to 195.71±56.38,there was no obvious difference between the 2groups in the changes of blood platelet,P>0.05,and it was not statistically significant,and there was no occurrence of intracranial hemorrhage,hemorrhage in digestive tract,urinary system and mucosa,etc,and there was no anaphylaxis.Conclusion:It is more effective to treat TCIA with clopidogrel and aspirin,it is safe and there is no toxic and side effect,it is an ideal medication mode for the treatment of transient cerebral ischemic attacks.
