良性前列腺增生症术前尿动力学检测的临床意义

目的探讨尿动力学检查在良性前列腺增生症(Benign prostatic hyperplasia,BPH)诊断和治疗中的意义。方法对151例BPH患者术前行尿动力学检查并进行尿动力学评估,为BPH患者的治疗提供理论依据。结果 151例患者中BOO梗阻程度0~Ⅲ级,逼尿肌收缩强度为(VW^W-)患者20例(占13.2%),合并膀胱活动过度5例,BOO梗阻程度0~Ⅱ级,逼尿肌收缩强度(N-~N+)患者8例(占5.3%),BOO梗阻程度Ⅲ~Ⅴ级,逼尿肌收缩强度(W+~N+)患者123例(占81.5%)。合并膀胱过度活动症38例(占30.9%),膀胱顺应性减低11例(占7.28%)。结论 BPH术前尿动力学检查,能客观准确评价BOO梗阻程度和逼尿肌功能,并对治疗方法的选择、术后疗效的评价、并发症原因的分析均有重要的价值。

Objective To investigate the significance of Urodynamics examination in the diagnosis and treatment of benign prostatic hyperplasia.Methods The preoperative urine kinetics and urine kinetic assessment of 151 patients with BPH were evaluated,theoretical basis for the treatment of BPH patients.Results 151 patients BOO obstruction degree 0-Ⅲ grade,20 patients with detrusor strength VW-W-,with overactive bladder in 5 cases,BOO obstructive degree 0-Ⅱ grade and detrusor strength for N--N＋ from8 patients,BOO degree of obstruction of theⅢ-Ⅴ grade,patients with detrusor strength W＋-N＋ 123 cases.38 cases were complicated with bladder hyperactivity,and 11 cases decreased bladder compliance.Conclusions BPH preoperative urodynamic examination,objective and accurate evaluation BOO the severity of obstruction and detrusor function,and for treatment selection,postoperative effect evaluation,the causes of complications of analysis have important value.
