内镜下结扎术治疗食管静脉曲张急性出血的疗效研究

Research of curative effect by endoscopic ligation in the treatment of esophageal varices acute hemorrhage

目的探讨内镜下结扎术在治疗食管静脉曲张急性出血中的疗效。方法 100例食管静脉曲张急性破裂出血患者,随机分为两组,每组50例。对照组患者采取内科药物治疗或加用三腔二囊管压迫止血;治疗组采用内镜下结扎术治疗出血。观察两组患者的治疗效果。结果治疗组的总止血率为94.00%,明显高于对照组的60.00%,且治疗组平均住院时间(2.98±0.42)d明显短于对照组的(5.34±0.28)d,治疗组再出血率4.00%低于对照组的56.00%,差异均有统计学意义(P<0.05)。结论内镜下结扎术治疗食管静脉曲张急性出血的疗效确切,急诊止血率高,住院时间短,并发症少,是一种安全、有效的治疗手段,值得推广应用。

Objective To investigate curative effect by endoscopic ligation in the treatment of esophageal varices acute hemorrhage. Methods A total of 100 patients with esophageal varices acute rupture hemorrhage were randomly divided into two groups, with 50 cases in each group. The control group received internal medicine therapy or in addition with compression hemostasis by Sengstaken-Blakemore tube. The treatment group received endoscopic ligation for hemostasis. Curative effects of the two groups were observed. Results The treatment group had obviously higher total hemostasis rate as 94.00% than 60.00% of the control group. The treatment group had much shorter average hospital stay time as（2.98±0.42） d than（5.34±0.28） d of the control group. The treatment group also had lower rehemorrhagia rate as 4.00% than 56.00% of the control group. Their differences all had statistical significance（P〈0.05）. Conclusion Endoscopic ligation provides precise effect in treating esophageal varices acute hemorrhage, along with high emergency hemostasis rate, short hospital stay time, and few complications. It is a safe and effective treatment measure, which is worth promoting and applying.
