经典方丝弓结合种植体支抗治疗骨性Ⅱ类高角错(牙合)畸形病例 被引量:1

The treatment of adolescent high angle Class Ⅱ malocclusion cases by classic directional-force edgewise technique with minicrews

摘要 目的 探讨经典方丝弓定向力矫治技术结合种植体支抗治疗青春期骨性Ⅱ类高角错(牙合)畸形的疗效.方法 应用经典方丝弓定向力矫治技术对14例处于青春期的患者进行正畸治疗,均拔除四颗第一双尖牙,使用高位J钩牵引头帽辅助内收前牙,并在上后牙区使用种植体主动压低上后牙,矫治前后拍摄头颅侧位X线片及测量分析,对矫治前后软硬组织头影测量结果进行配对t检验.结果 14例患者的下颌均有明显的生长,矫治后FMA角由(32.1±5.1)°改善为(29.5±4.6)°,ANB角由(6.4±2.0)°减少到(3.7±1.4)°.治疗后面型得到明显改善.结论 定向力矫治技术结合种植体支抗能有效的垂直向控制上下颌骨的生长,引导下颌骨的逆时针旋转,使Ⅱ类高角患者面型得到极大改善. Objective To investigate the therapeutic effects of classic directional-force edgewise technique with Minicrews in treatment of adolescent Class Ⅱ high angle cases.Methods 14 adolescent patients with four first premolar extractions were included in the study.They were provided with high-pull J-hook anterior retraction and posterior miniscrew intrusion treatment.Cephalograms of pretreatment(T1) and posttreatment (T2) were measured.The changes of the soft and hard tissues were evaluated by paired t-test.Results All the 14 patients presented obvious mandibular growth,with FMA decreased from (32.1±5.1)° to (29.5±4.6)° and ANB decreased from (6.4±2.0)° to (3.7 ± 1.4)°.Profiles were also improved significantly.Conclusions The combination of the directional-force technique and Minicrews, can control the vertical growth both of the maxilla and mandible, and stimulate the growth potential of the mandible and counterclockwise rotate the mandible to improve the profile.
