某院2009年-2014年首诊卵巢癌住院患者特征分析

Analysis on Characteristics of Inpatients with Primary Diagnosed Ovarian Cancer in a Hospital from 2009 to 2014

目的分析某院首次就诊的卵巢癌住院患者基本特征的动态变化,为优化卵巢癌防治相关卫生资源配置提供指导。方法资料来自于2009年-2014年1543份卵巢癌住院患者的病案首页数据,包括基本人口统计学特征和临床资料等信息,描述各种特征值的变化趋势。结果卵巢癌住院患者的年龄主要分布在41岁~70岁,尤以51岁~60岁者最多;病理类型主要为浆液性癌和未分型癌;74.74%患者的癌症分化程度为未分化/低分化;手术治疗的比例为45.37%;87.04%的患者住院天数≤20天,51.52%的患者住院天数≤10天。卵巢癌住院患者的癌症分化程度（χ~2=4.40,P〈0.05）和住院天数（χ~2=7.16,P〈0.05）随年份的变化有统计学意义,其他各特征值的变化趋势均无统计学意义。结论未分化/低分化、住院天数为10天及以下的卵巢癌住院患者有逐渐增多的趋势,医院应着重完善对这两类患者的诊断、治疗与护理的一体化服务。

Objective To investigate the dynamic changes of characteristics of inpatients with primary diagnosed ovarian cancer in a hospital for providing guidance for the optimization of health resources allocation in the prevention and treatment of ovarian cancer. Methods The data of medical records for 1,543 inpatients with primary diagnosed ovarian cancer were collected from 2009 to 2014, including some general and clinical information of individuals. The trends of these characteristic values were described. Results The age of inpatients with primary diagnosed ovarian cancer was mainly 41-70 years old, especially 51~60 years old. Pathological type was mainly serous carcinoma and unclassified carcinoma. There were 74.74% of inpatients with undifferentiated or poorly differentiated ovarian cancer, and 45.37% of inpatients who had been treated with operation. There were 87.04% of inpatients who had stayed in hospital for 20 days or above, and more than half（51.52%） of inpatients whose hospital day was 10 days or below. The differentiation degree of ovarian cancer（χ2=4.40, P〈0.05） and the hospital day of inpatients（χ2=7.16, P〈0.05） showed significantly changes with years. However, the other changes of characteristics of inpatients were not statistically significant. Conclusion There is a gradual increase in the number of inpatients with undifferentiated or poorly differentiated ovarian cancer and with 10 hospital days or below. Thus, the hospital should focus on improving the integrated services including diagnosis, treatment and nursing care for the two kinds of ovarian cancer inpatients.
