Analysis of Ding Ganren's Experiences of Treating Diarrhea

[目的]探析丁甘仁辨治泄泻经验。[方法]研读《孟河丁甘仁医案》中有关辨治泄泻的医案,从病因病机、治法、用药特色等方面,对丁氏治疗泄泻进行解析。[结果]丁氏认为泄泻以脾虚湿盛为基本病机,十分重视肝肾在泄泻中的作用,治疗以健脾化湿为基础,注重抑肝扶脾、益火补土,治法涉及芳香化湿、淡渗利湿、抑肝扶脾、益火扶土、和中分利、利小便实大便、逆流挽舟、解表化湿、固涩等各种方法,善用党参、白术、山药以健脾,茯苓、泽泻、薏苡仁以利湿,藿香、佩兰、半夏、陈皮苦温燥湿,喜用白芍、木瓜、乌梅、防风以柔肝抑肝,常用附子、肉桂、补骨脂、益智仁以温肾助阳,尤其善用荷叶以升阳止泻。[结论]丁氏对泄泻辨证独到,用药轻灵,能融汇古今,博采众长,值得学习。

[Objective] Analysis of Ding-Genre＇s experiences of treating diarrhea. [Method] Study related medical records on treating diarrhea from the ＂Ding Ganren Medical Records＂ ,then analysis of treating diarrhea of Ding from pathogenesis, therapy, medication and other aspects.[Results] Ding believed that the basic pathogenesis of diarrhea due to spleen dampness, and attached great importance to the role of the liver and kidney in diarrhea. The spleen and dampness as the foundation in the treatment,Ding paid attention to restric, ting liver and supporting spleen, warming the kidney and strengthening the spleen. The therapy involves eliminating dampness with aromatics, diuresis-removing dampness,supporting spleen, restricting liver,warming the kidney and strengthening the spleen, Hezhongfenli, urinating to stop diarrhea, hauling the boat upstream, relieving exterior and dissipating dampness, astringents etc. tie made the best use of codonopsis, atractylodes, yam to invigorate the spteen, used the poria, alisma, coix to remove dampness, used the patchouli, perrin, pinellia, dried orange peel to bitter and warm dampness, liked to use white peony, papaya, plum, radix saposhnikoviae restricting and nourishing liver, commonly used aconite, cinnamon, fructus psoraleae, puzzle kernel to warm kidney, especially made the best use of lotus leaf invigorating splenic yang and stop diarrhea. [Conclusion] Ding uniquely discriminated diarrhea, c, laimed mild treatment and gentle application of the medicine, integrated ancient and modern, learnt widely from others＂ strong points, which is worth learning.
