剪切波弹性成像技术对颈动脉粥样斑块稳定性的初步评估 被引量:1

摘要 目的 :采用剪切波实时组织弹性成像技术(shear wave elastography,SWE)探讨其对颈动脉粥样斑块稳定性的应用价值。方法:107例颈动脉粥样斑块患者分为脑梗死组(56例)与非脑梗死组(51例),应用彩色多普勒超声仪分别观测两组颈动脉斑块类型及斑块形态,应用剪切波弹性成像技术观察不同斑块的弹性图像特点并测得斑块平均弹性模量值(SWE1)、斑块近心端弹性模量值(SWE2)及两者的比值(SWE1/SWE2)。结果:1脑梗死组颈动脉斑块以脂质型斑块(60.7%)为主,非脑梗死组以混合型斑块(47.1%)为主,脑梗死组斑块表面呈不规则溃疡型较非梗死组明显增多(P<0.05);2超声弹性图像中,脂质型斑块表现为蓝色、黄色为主;混合型斑块表现为不均匀黄色、红色或不规则黄红相间;强回声钙化回声斑块表现为红色为主;3脑梗死组斑块的SWE2明显低于非梗死组,而SWE1/SWE2则明显增高,差异具有统计学意义(P<0.05),两组间SWE1比较,差异无统计学意义。结论:剪切波弹性成像技术可以观察斑块的弹性图像特点及定量测得各种斑块弹性模量值,可为动脉粥样硬化斑块稳定性的评估提供更多的信息。
