医院社区联动自我管理教育模式在88例糖尿病患者护理中的应用效果观察

Application Effect Observation of Hospital Community Linkage Self Management Education Mode in 88 Patients With Diabetes Mellitus

目的探究分析医院社区联动自我管理模式在糖尿病护理中的应用效果。方法按照不同的教育方式将176例糖尿病患者分组进行对照研究,分别给予医院社区联动自我管理教育和常规教育健康教育方式。对比分析两组治疗前后空腹血糖、糖尿病掌握情况、自我管理能力等方面。结果治疗前两组空腹血糖差异较小,治疗后观察组血糖更低,糖尿病健康知识掌握情况更好,自我管理更优(P<0.05)。结论医院社区联动自我管理教育模式是糖尿病护理首选的教育方式。

Objective To explore the application effect of the hospital community linkage self management mode in the diabetes care. Methods 176 patients with diabetes were divided into the observation group and the control group according to the different educational methods, and the hospital community was given the self management education and general education. Comparative analysis of two groups of patients before and after treatment of fasting blood glucose, diabetes, self-management. Results The difference of fasting blood glucose between the two groups was lower, and the observation group was significantly lower than that in the observation group, and the health knowledge of diabetes mellitus was much better, and the self management was better（P〈0.05）. Conclusion The hospital community linkage self management education model is the first choice for diabetes care education.
