南宁市兴宁区城乡2型糖尿病患者社区管理效果评价

Effect evaluation on community management of patients with type 2 diabetes in urban and rural areas of Xingning District of Nanning City

目的了解南宁市兴宁区接受社区管理满1年以上的糖尿病患者血糖控制情况及其影响因素,为改善管理措施提供参考依据。方法在辖区内已经接受基本公共卫生服务管理的2型糖尿病患者中,随机抽取369例为本次研究的对象。调查其患病情况、糖尿病相关知识、生活行为方式、接受糖尿病管理服务以及身体测量等内容。结果城市患者血糖监测频率高于农村患者(P<0.01);城市患者发生并发症(57.1%)的高于农村患者(40.0%)(P<0.01);城市患者对血糖正常值、糖尿病常见症状、糖尿病相关危险因素、糖尿病常见并发症的知晓率均高于农村患者(P<0.01);服药依从性上,城市患者(75.7%)好于农村患者(36.7%);城市患者血糖控制率(55.0%)低于农村患者(67.0%)。结论在该区慢性病社区管理工作中,城市患者知晓率、监测频率和治疗依从性更高;患者不良健康行为和血糖控制方面,仍未取得满意效果。为了提高患者的健康水平和生活质量,达到慢性病管理的最终效果,仍需要完善当前的管理模式。

[Objective]To know the blood glucose control status of diabetes patients who have received the community management for one year in Xingning District of Nanning City,analyze the influencing factors,and provide the basis for improving the management measures.[Methods]369 patients with type 2 diabetes were randomly sampled from patients who have received the basic public health service management in Xingning District.The investigation contents included disease situation,diabetesrelated knowledge,lifestyle behaviors,receiving status of diabetes management services,as well as physical measurements.[Results]The blood glucose monitoring frequency of urban patients was higher than that of rural patients（P〈0.01）,and the incidence rate of complications in urban patients（57.1%） was higher than that of rural patients（40.0%,P〈0.01）.The awareness rates on normal value of blood glucose,common symptoms of diabetes,risk factors of diabetes,and common complications of diabetes in urban patients were higher than those in rural patients（P〈0.01）.The urban patients had the better medication compliance（75.7%） than the rural patients（36.7%）,while the blood glucose control rate of urban patients（55.0%） was lower than that of rural patients（67.0%）.[Conclusion]In the community management of chronic diseases,the awareness rates,monitoring frequency and compliance in urban patients are higher than those in rural patients.Meanwhile,the adverse health behaviors and blood glucose control among patients have not obtained satisfactory results.In order to enhance the health level and quality of life of patients,and achieve the better effect of chronic disease management,the current health management mode needs to be improved.
