摘要
目的探讨负压封闭引流技术(vacuum sealing drainage,VSD)在创伤感染患者治疗中的临床应用价值。方法我们收集了25例创伤伴感染患者,将他们随机分成两组:VSD组(13例)和对照组(12例)。两组患者均接受清创、抗感染和对症治疗,其中VSD组接受VSD治疗,对照组接受常规换药治疗。比较两组每日换药次数、住院期间总换药时间(分钟)、平均每日住院费用。同时比较VSD组术前、术后24和48小时C反应蛋白(CRP)、降钙素原(PCT)和白细胞(WBC)水平。结果 VSD组较对照组换药次数(次/天)、住院期间总换药时间(分钟)减少,VSD组较对照组平均每日住院费用(元/天)增加(P>0.05)。VSD组患者术后48小时CRP、PCT和WBC值与术前比较差异有统计学意义(P<0.05)。结论 VSD在下肢创伤感染患者治疗中具有良好的性价比,值得临床推广使用。
Objective To investigate the clinical application value of VSD( vacuum sealing drainage)technique in the treatment of patients with traumatic infections. Methods We collected 25 patients with lower extremity trauma infection and divided them into two groups. One group including 13 cases was treated with VSD( VSD group),and the other group( control group) including 12 cases was treated with traditional dressing change methods. Both groups were treated with debridement,anti- infection and symptomatic treatment. The times of daily dressing change,total time of change the dressing during hospitalization( minutes),the average daily cost of hospitalization of the two groups were compared. Simultaneously,the concentration of serum C-reactive protein,Procalcitonin and White blood cell of VSD group before the therapy,24 and48 h after the treatment were also compared. Results The times of daily dressing change( / day) and total time of change the dressing during hospitalization( minutes) of VSD group were significantly decreased compared with those of the contrast group( P〈0. 05). The average daily cost of hospitalization of VSD group were increased compared with that of the contrast group( P〈0. 05). We observed statistically significant decreases( P〈0. 05) in the serum levels of C- reactive protein,Procalcitonin and White blood cell of VSD group in 48 h after the treatment compared with those before the therapy. Conclusions The use of VSD in lower extremity trauma infection had an excellent ratio between effects and values and should be recommended to use and develop for clinical application.
