多巴酚丁胺对新生儿肺动脉高压患儿C-反应蛋白、D-二聚体、BNP水平及血气分析影响研究

Study effect of dobutamine on C- reactive protein,D- dimer,BNP and blood gas analysis in children with neonatal pulmonary hypertension

目的探讨多巴酚丁胺对新生儿肺动脉高压患儿C-反应蛋白、D-二聚体、BNP水平及血气分析的影响。方法收集江西省九江市妇幼保健院收治的新生儿持续性肺动脉高压患者46例,随机分为对照组和实验组,每组各23例,对照组给予镇静、维持水电解质平衡、血压、抗感染等常规治疗,实验组在对照组基础上给予盐酸多巴酚丁胺注射液5μg/(kg·min)静脉泵入,连续48 h。治疗结束后,对所有患儿的外周血C-反应蛋白、D-二聚体、BNP水平及血气分析情况进行检测。结果与对照组治疗后比较,实验组患儿的血清CRP水平、血浆D-二聚体水平、BNP水平均较低(P<0.05);实验组患儿的Pa O2、Sp O2水平明显较高,PAH水平明显较低(P<0.05)。结论多巴酚丁胺能够显著降低新生儿肺动脉高压患儿血清CRP、血浆D-二聚体以及BNP水平,降低肺动脉高压。

Objective To analyse effect of dobutamine on C- reactive protein,D- dimer,BNP and blood gas analysis in children with neonatal pulmonary hypertension. Methods 46 patients who were diagnosed with neonatal pulmonary hypertension in our hospital were collected. All patients were randomly divided into experimental group and control group,23 cases in each group. Control group was treated with sedation,maintenance of water electrolyte balance,blood pressure,anti-infection,experiment group was treated with dobutamine hydrochloride injection 5 μg /（ kg·min） intravenous infusion,continuous for 48 h on the basis of conventional therapy,after treatment,the levels of peripheral blood C- reactive protein,D- dimer and BNP and blood gas analysis effect were detected in all patients. Results Compared with control group post-treatment,the serum level CRP,D- dimer,BNP were lower in experimental group（ P〈0. 05）; the Pa O2 and Sp O2 levels were higher,the PAH level was lower in experimental group（ P〈0. 05）.Conclusion The dobutamine can significantly reduce the serum CRP,plasma D-dimer and BNP levels in children with neonatal pulmonary hypertension,reduce pulmonary hypertension.
