摘要
目的:比较肝脏硬度测定仪FibroTouch和FibroScan对肝纤维化的诊断价值及其与肝脏病理分期间的相关性。方法:选取2013年1月至2015年1月我院临床基本信息和肝脏病理资料齐全的81例肝病患者,分别用FibroTouch和FibroScan2种仪器同时测定其肝纤维化程度,比较2种仪器检测结果间的一致性,并分析FibroTouch检测结果与肝脏病理组织分期间的一致性。结果:FibroTouch和FibroScan检测81例患者肝纤维化程度的总平均值分别为(10.19±7.16)kPa和(9.90±7.89)kPa,差异无统计学意义(t=0.681,P=0.498);且相关性分析显示,2种检测的结果间呈正相关,相关系数为0.772(P<0.05)。以肝组织病理纤维化分期结果为依据,FibroTouch诊断肝纤维化≥S1的受试者工作特征(ROC)曲线下面积为0.442;诊断肝纤维化≥S2的ROC曲线下面积为0.687;诊断肝纤维化≥S3的ROC曲线下面积为0.824;诊断肝纤维化≥S4的ROC曲线下面积为0.869。结论:FibroTouch检测评估肝脏纤维化程度的价值较高,且其结果与FibroScan结果及肝组织病理诊断纤维化分期间一致性较好。
Objective: To compare the diagnostic value of hardness testing instruments Fibro Touch and Fibro Scan in patients with liver fibrosis and to investigate their correlation with liver pathological staging. Methods: Eighty-one patients with pathologically comfirmed liver disease during Jan. 2013 and Jan. 2015 were enrolled. FibroTouch and Fi-broScan were used to evaluate the degree of liver fibrosis, and liver biopsy was also performed. The results of Fibro Touch and FibroScan and pathological examination of biopsy specimen were analyzed by statistical analysis to assess the consis-tency between Fibro Touch and FibroScan and their correlation with histological finding. Results: Analysis showed that the results from Fibro Touch were not significantly different from that of Fibro Scan(t=0.681, P=0.498), and the correlation coefficient of Pearson'S correlation analysis was 0.772(P0.05), suggesting that results of the two technology were in consistency with each other. For the histopathology staging of liver fibrosis, the diagnosis by Fibro Touch for liver fibrosis ≥S1had a receiver operating characteristic(ROC) area under the curve(AUC) of 0.442,for liver fibrosis ≥S2 had a ROC AUC of 0.687, for liver fibrosis ≥S3 had a ROC AUC of 0.824, and for liver fibrosis ≥S4 had a ROC AUC of 0.869.Conclusions: Fibro Touch is also an effective diagnosis method for detecting liver fibrosis, and it is in consistency with the liver pathological staging determined by histopathology.
