Glisson蒂横断式精准肝切除联合美蓝染色法在肝癌中的应用价值

Value of glisson pedicle transection precise hepatectomy combined with methylene blue staining method in application of liver cancer

目的探讨Glisson蒂横断式精准肝切除联合美蓝染色法应用于肝癌患者的临床优势。方法将我院2011年1月~2013年12月收治的121例肝癌患者分组,其中对照组60例给予传统Prigle法治疗,观察组61例予以Glisson蒂横断式精准肝切除联合美蓝染色法治疗,对比两组疗效。结果两组手术时间与术中出血量,以及住院时间与费用组间差异较大,且观察组各项明显更理想（P〈0.05）;观察组术后AST、ALT指标均明显较对照组低（P〈0.05）;术后观察组并发症与复发率分别为9.84%与13.11%,对照组为31.67%与35.00%,组间比较差异有统计学意义（P〈0.05）。结论将Glisson蒂横断式精准肝切除联合美蓝染色法应用于临床对肝癌患者的治疗,疗效确切,且并发症率与复发率低,该法深具临床推广价值。

Objective To explore clinical advantages of Glisson pedicle transection precise hepatectomy combined with methylene blue staining method in application of patients with liver cancer. Methods 121 patients with liver cancer who were admitted to our hospital from January 2011 to December 2013 were allocated to the control group with 60 cases and the observation group with 61 cases.The control group was received treatment of traditional Prigle method while the observation group was received treatment of Glisson pedicle transection precise hepatectomy combined with methylene blue staining method.Curative effects of two groups were compared. Results Operation time, intraoperative blood loss,hospital time and hospital expenses of two groups had great differences.But every index in the observation group was more ideal（P〈0.05）. AST and ALT indexes after operation of the observation group were significantly lower than those of the control group（P〈0.05）. Complication rate and recurrence rate after operation of the observation group were 9.84% and 13.11% respectively.Complication rate and recurrence rate after operation of the control group were 31.67% and 35.00% respectively. There were significant differences between groups（P〈0.05）. Conclusion Glisson pedicle transection precise hepatectomy combined with methylene blue staining method in clinical treatment of patients with liver cancer has accurate curative effect and low complication rate and recurrence rate, which is of clinical promotion value.
