卡前列甲酯预防无宫缩剖宫产术后出血的临床研究

Clinical study on sublingual carboprost methylate prevention without uterine hemorrhage after cesarean section

目的探讨卡前列甲酯预防无宫缩剖宫产术后出血的临床疗效。方法选择2011年1月~2013年12月笔者所在医院收治剖宫产单胎孕妇260例,随机分为观察组与对照组,每组130例。两组患者均常规使用缩宫素,观察组产妇术中胎肩娩出后,立即舌下含服卡孕栓1mg。比较两组患者产后出血量及副作用。结果观察组患者术中、术后2h出血量均明显少于对照组,差异有统计学意义（P〈0.05）;而两组患者术后2~24h出血量无统计学意义（P〉0.05）。观察组产后出现发生率3.85%,明显低于对照组12.31%,具有统计学意义（P〈0.05）。结论舌下含服卡前列甲酯预防无宫缩剖宫产术后出血具有安全有效、方便、简单的特点。卡前列甲酯协同缩宫素作用使子宫收缩明显加强,产后出血显著减少,是一种预防产后出血的有效方法。

Objective To explore the clinical curative effect of Sublingual carboprost methylate prevention without uterine hemorrhage after cesarean section. Methods 260 cases of cesarean section,who were treated in the author＇s hospital from January 2011 to December 2013,were selected and were randomly divided into observation group and control group,130 cases in each group.Two groups of patients were given conventional oxytocin.Women of observation group were immediately given Carprost Suppositories 1mg.The amount of postpartum hemorrhage and the side effects of the two groups were compared. Results The postoperative 2h bleeding volume of the observation group was significantly less than that of the control group,the difference was statistically significant（P〈0.05）,and the two groups of patients with no significant difference in postoperative 2~24h bleeding volume（P〉0.05）.The occurrence rate of the observation group was 3.85%,which was significantly lower than that of the control group（12.31%） with statistical difference（P〈0.05）. Conclusion Sublingual carboprost methylate prevention without uterine hemorrhage after cesarean section has the characteristics of simple,convenient, safe and effective.The synergistic effect of methyl ester on the uterine contraction was significantly enhanced, and postpartum hemorrhage was significantly decreased,which is an effective method for preventing postpartum hemorrhage.Methyl ester combined with oxytocin can significantly strengthen the uterine contraction, significantly reducing postpartum hemorrhage is a kind of effective method for prevention of postpartum hemorrhage.
