血清降钙素原在危重患者细菌感染检测中的应用

目的分析血清降钙素原在危重患者细菌感染检测中的应用价值。方法选择2011年1月~2014年12月我院ICU收治的危重症感染患者101例作为观察组,选取同期健康体检者100例作为对照组,分析PCT、hs CRP及WBC的检测结果。结果观察组患者PCT、hs CRP及WBC检测结果均较对照组明显增高,且PCT检测阳性率明显高于血培养（P〈0.05）,而53例血培养阳性的患者,PCT检测结果均为阳性,符合率为100%。PCT诊断危重症感染的敏感性与特异性均显著高于hs CRP、WBC（P〈0.05）。PCT水平在三组患者中随着感染程度加重呈现出递增趋势（P〈0.05）。结论血清降钙素原检测方便,灵敏度高、特异性好,可作为危重患者细菌感染的监测指标。

Objective To analyze the application value of procaleitonin detection in critical illness patients with bacterial infection. Methods 101 critical illness patients with bacterial infection in ICU of our hospital from January 2011 to December 2014 were selected as the observation group,100 healthy human with physical examination during the same period were selected as the control group,the detection results of PCT,hs CRP and WBC were analyzed. Results The levels of PCT,hs CRP and WBC in the observation group were significantly higher than those in the control group,and the positive rate of PCT was significantly higher than that of blood culture（P〈0.05）,and the PCT of 53 patients with positive blood culture was positive,the coincidence rate of both was 100%.The sensitivity and specificity of PCT in the diagnosis of critical illness patients with infection were significantly higher than that of hs CRP and WBC（P〈0.05）.With the increase of infection degree,the level of PCT increased in the three groups of patients（P〈0.05）. Conclusion Procaleitonin detection is convenient,and has high sensitivity and specificity,and can be used as a monitoring index for critical illness patients with bacterial infection.
