医院-社区-家庭护理模式老年冠心病合并高血压40例

Community- Hospital- Home Care Model for the Elderly Coronary Artery Disease Combined with Hypertension in 40 Cases

目的观察医院-社区-家庭护理模式在硝苯地平联合依那普利治疗冠心病合并高血压老年患者中的临床效果。方法选取在医院行硝苯地平联合依那普利治疗冠心病合并高血压老年患者80例,按随机数字表法分为研究和对照组,各40例。研究组患者采用医院-社区-家庭护理模式对患者进行长达1年的院外干预,对照组患者采用常规护理方法。结果干预1年后,研究组患者收缩压(SBP)及舒张压(DBP)明显低于对照组(P<0.05);研究组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)及知觉压力量表(CPSS)评分明显低于对照组(P<0.05);研究组满意率明显高于对照组(P<0.05);研究组复发率明显低于对照组(P<0.05)。结论采用医院-社区-家庭护理模式应用于硝苯地平联合依那普利治疗冠心病合并高血压老年患者疗效明显,大大降低了患者血压,缓解了冠心病合并高血压的发生,值得推广。

Objective To study the community- hospital home care model in nifedipine combined with enalaprilat in treating hypertension in the elderly patients with coronary artery disease. Methods 80 cases of elderly patients with coronary heart disease combined with from January 2013 to December 2014 with the treatment by nifedipine combined with enalaprilat was selected and divided into the study group and the control group according to random number table method,40 cases in each group. The study group applied the hospital- community- family nursing mode for up to 1 year of outside intervention, and the control group received routine nursing methods. Results After 1 year of intervention,the SBP and DBP of the study group were significantly lower than those of the control group（ P 〈 0. 05）; the SAS, SDS and CPSS scores of the study group were significantly lower than those of the control group（ P 〈 0. 05）;the patients satisfaction rate of the study group was significantly higher than the control group（ P 〈 0. 05）; the recurrence rate of the study group was significantly lower than the control group（ P 〈 0. 05）. Conclusion The hospital- community- home care model used in nifedipine combined with enalaprilat in the treatment of elderly coronary heart disease combined with hypertension has obvious effect,greatly reduces the blood pressure of patients,alleviates the incidence of coronary heart disease with hypertension,which is worthy of clinical promotion.
