鼓膜ADM修复贴膜与自然愈合对外伤性鼓膜穿孔影响的对比研究

Comparative study of tympanic membrane repair ADM film and natural healing effect on traumatic perforation with tympanic membrane

目的探讨鼓膜ADM修复贴膜对外伤性鼓膜穿孔的临床价值。方法选择2011年1月-2014年12月我院收治的外伤性鼓膜穿孔患者80例,随机分为两组,各40例。观察组应用鼓膜贴膜材料为A型异种（牛）ADM修复膜;对照组采用保守治疗,促使患者鼓膜穿孔自然愈合。两组患者均通过门诊随访3个月,比较两组鼓膜愈合时间、愈合后高频平均听阈,统计治疗过程中发生的并发症及临床效果。结果观察组鼓膜愈合时间短于对照组（P〈0.05）;愈合后高频平均听阈低于对照组（P〈0.05）;观察组治疗过程中发生感染、鼓膜血肿、鼓膜不良愈合及耳鸣耳聋的比例显著低于对照组（P〈0.05）;观察组总有效率高于对照组（P〈0.05）。结论 ADM鼓膜贴膜是一种理想的鼓膜穿孔修复材料,适合于基层医院推广。

Objective To investigate the comparative study of tympanic membrane repair ADM film and natural healing effect on traumatic perforation with tympanic membrane. Methods From January 2011 to December 2014,80 patients with traumatic perforation of the membrane were treated in our hospital were randomly divided into two groups,each 40 cases.The observation group were used tympanic membrane foil type a heterogeneous material（cattle） ADM repair film,the control group were used conservative treatment to promote natural healing,all patients were followed-up with three months,then tympanic membrane healing time,high-frequency average hearing threshold occurrence of complications and clinical results,statistical treatment during healing between two groups was compared respectively. Results The tympanic membrane healing time with the observation group was shorter than control group（P〈0.05）,high-frequency hearing threshold healing was lower than control group（P〈0.05）,infection occurs tympanic hematoma,adverse tympanic membrane healing and tinnitus deafness the proportion with observation group was lower than control group respectively（P〈0.05）,the effective power was higher than control group（P〈0.05）. Conclusion ADM is an ideal foil eardrum perforated eardrum repair material for tympanic membrane.It is suitable for primary hospitals.
