针刀整体松解术配合手法推拿治疗腰椎间盘突出症

Needle- Knife Overall Release Combined with Massage in Treatment of Lumbar Disc Herniation

目的:针刀整体松解术配合手法推拿对腰椎间盘突出症患者神经炎性因子及治疗效果的影响。方法:将2013年9月—2014年9月期间本院收治的90例腰椎间盘突出症患者纳入研究对象,采用随机数字表法分为观察组和对照组各45例,观察组患者接受针刀整体松解术配合推拿治疗,对照组接受单纯推拿治疗,比较两组患者的腰椎功能以及炎性因子含量。结果:1腰椎功能与疼痛评分:观察组JOA评分明显高于对照组(24.18±3.24)vs(18.21±2.25)(P<0.05),ODI、VAS评分明显低于对照组(14.12±1.81)vs(22.32±3.32),(2.82±0.45)vs(4.05±0.53)(P<0.05);2腰部活动度:观察组前屈、后伸、侧屈活动度均明显高于对照组(85.32±4.52)vs(76.23±4.32),(25.56±3.34)vs(20.05±3.12),(26.12±2.42)vs(21.04±2.36)(P<0.05);3炎性因子:观察组血清中TNF-α、IL-1、IL-6和IL-8的含量低于对照组[(17.15±2.27)vs(29.62±3.36)ng/m L,(9.39±1.03)vs(14.59±2.23)pg/m L,(31.18±4.32)vs(58.74±7.83)pg/m L,(123.62±21.29)vs(231.45±34.43)pg/m L](P<0.05)。结论:针刀整体松解术配合手法推拿有助于改善腰椎功能和活动度,缓解疼痛程度和炎症反应程度,提高治疗效果。

Objective： To study the effect of needle- knife overall release therapy combined with massage on nerve inflammatory factors and the treatment effect on senile lumbar disc herniation patients. Methods： 90 cases patients with senile lumbar disc herniation in our hospital from May 2012 to May 2014 were enrolled and randomly divided into observation group and control group. Observation group received massage combined with needle- knife overall release therapy and the control group received simple massage therapy. The lumbar function and inflammation factors contents were compared. Results： 1 Lumbar function and pain scores： observation group＇s JOA score was significantly higher than that of control group [（ 24. 18 ± 3. 24） vs（ 18. 21 ± 2.25） 〗（ P〈0.05）. ODI and VAS scores were significantly lower than those of the control group[（ 14. 12 ± 1. 81） vs（ 22. 32 ± 3.32）,（ 2. 82 ± 0. 45） vs（ 4. 05 ± 0. 53）（ P〈0.05） ]. 2Lumbar motion： observation group＇ s forward and backward bends and lateral flexion activity were significantly higher than those of the control group [（ 85. 32 ± 4. 52） vs（ 76. 23 ± 4. 32）,（ 25. 56 ±3. 34） vs（ 20. 05 ± 3. 12）,（ 26. 12 ± 2. 42） vs（ 21. 04 ± 2. 36）（ P〈0.05） ]. 3 Inflammation factors contents： observation group＇s serum TNF- α,IL- 1,IL- 6 and IL- 8 contents were significantly lower than those of control group [（ 17. 15 ± 2. 27）vs（ 29. 62 ± 3. 36） ng / m L,（ 9. 39 ± 1. 03） vs（ 14. 59 ± 2. 23） pg / m L,（ 31. 18 ± 4. 32） vs（ 58. 74 ± 7. 83） pg / m L,（ 123. 62 ±21. 29） vs（ 231. 45 ± 34. 43） pg / m L]（ P〈0.05）. Conclusion： Massage combined with needle- knife overall release therapy is helpful to improve treatment compliance,the lumbar vertebral function and activity and alleviate pain degree and inflammation reaction degree. It＇s an ideal method in treating lumbar disc herniation.
