社会因素对剖宫产后瘢痕子宫再次妊娠产妇分娩方式的影响

Analysis of the Influences of Social Factors on Delivery Mode Choice of Secondary Pregnancy with Cicatricial Uterus after Cesarean Section

目的观察社会因素对剖宫产后瘢痕子宫再次妊娠产妇分娩方式选择的影响。方法收集来咸宁市妇幼保健院就诊临床诊断为剖宫产后瘢痕子宫并再次妊娠待产的孕妇400例,按其在分娩前愿意采取的分娩方式将其分为两组,经阴道分娩组与再次行剖宫产组,分析其一般状况及各种社会因素对其影响,并比较两组对剖宫产近远期风险的了解情况。结果对剖宫产后瘢痕子宫再次妊娠产妇分娩方式选择影响最大的因素为恐惧宫缩痛,再次行剖宫产组为70.0%,而阴道分娩组为35.0%,两组之间的差异具有统计学意义(P<0.05);再次行剖宫产组孕妇对近期危害的了解率为52.1%,对远期危害的了解率为32.5%,而阴道分娩组孕妇分别为75.6%和65.6%,两组孕妇在近期危害及远期危害方面的差异均有统计学意义(P<0.05)。结论社会因素对剖宫产后瘢痕子宫再次妊娠产妇分娩方式的选择具有一定的影响,值得临床医师关注。

Objective To observe the influences of social factors on delivery mode choice of secondary pregnancy with cicatricial uterus after cesarean section. Methods 400 cases of pregnant women were divided into vaginal delivery group and sencendary caerean section group by their chosen delivery modes,analysis the general status and potential social factors influenced on their choices and compare the understanding degrees of shor- term and long- term risks of caesaren section of both groups. Results Uterine contraction pain was the main factor on the choice of delivery mode of the secondary pregnancy with cicatricial uterus after cesarean section group,the rate was 70. 0%,the viginal delivery group was 35. 0%,the difference was statistical significant（ P〈0. 05）,the rates of the understanding of the short- term and long- term risks of the secondary pregnancy with cicatricial uterus after cesarean section group was 52. 1% and 32. 5%,the rates of vaginal delivery group were 75. 6% and 65. 6%,the difference was statistical significant（ P〈0. 05）. Conclution Social factors has a certain influence on delivery mode choice of secondary pregnancy with cicatricial uterus after cesarean section,is worthy of paying clinical attention.
