侧方蒂指腹旋转皮瓣修复手指末节软组织缺损

Side pedicle finger rotation flap for repairing finger soft tissue defect

目的观察切取侧方蒂指腹旋转皮瓣修复手指末节软组织缺损的治疗效果。方法应用侧方蒂指腹旋转皮瓣修复18例手指末节软组织缺损患者(19指),软组织缺损面积0.8 cm×1.0 cm^1.2 cm×1.5 cm,皮瓣切取面积1.0 cm×1.2 cm^1.4 cm×1.7 cm。结果 18例(19指)皮瓣全部成活。15例(16指)获得随访,时间4~15个月,3例失访。获得随访的患者指腹外形逼真,无瘢痕挛缩,肤色与正常组织相同,皮瓣弹性良好,无色素沉着,手指活动功能良好。皮瓣感觉良好,两点辨别觉为3~7 mm;耐寒冷,无寒冷性刺痛;皮肤有螺纹,可出汗。按中华医学会手外科学会上肢部分功能评定试用标准评定:15例患者(16指)功能均达优。结论侧方蒂指腹旋转皮瓣修复手指末节软组织缺损手术操作安全简单,血供充足,皮瓣外形逼真,活动及感觉功能恢复良好,对供区影响小。

Objective To observe the effect of side pedicle finger rotation flap for repairing finger soft tissue defect.Methods Side pedicle finger rotation flap was used to repair 18 patients（ 19 fingers） of soft tissue defect in the finger,soft tissue defect area was 0. 8 cm × 1. 0 cm - 1. 2 cm × 1. 5 cm,flap cut area was 1. 0 cm × 1. 2 cm - 1. 4 cm ×1. 7 cm. Results All 18 cases（ 19 fingers） flaps survived,15 patients（ 16 fingers） patients were received followup,the time was 4 - 15 months,3 cases were lost to follow-up. For follow-up of the appearance of the pulp was realistic,no scar contracture,the same color and normal tissue flap,good elasticity,no pigmentation,good function of finger movement. Flap feel was good,the two-point discrimination was 3 - 7 mm; Cold resistance,no cold sting;skin thread could be sweating. According to the Chinese medical association in to upper part features the probation evaluation standard： 15 patients（ 16 fingers） functions were optimal. Conclusions Side pedicle finger rotation flap for repairing finger soft tissue defect operation safe and simple operation,the blood supply is adequate,flap life like appearance,activity and sensory function recover well,little influence on the donor area.
