2009-2014年湖北省涂阳肺结核密切接触者检查情况分析

Analysis on close-contact screening of smear-positive PTB registered in Hubei(2009-2014)

目的分析涂阳肺结核患者密切接触者结核患病情况,为今后的密切接触者筛查工作提供可行性建议。方法 2009-2014年收集全省涂阳肺结核患者密切接触者检查季度报表、患者登记年报表和来源于密切接触者的患者病案信息,采用SPSS11.5软件对资料进行统计分析。结果 2009-2014年,全省共登记涂阳密切接触者275 069例,筛查271 427例,共检出活动性肺结核患者610例,其中涂阳肺结核患者242例;检出率分别为0.49%和0.19%。无症状者肺结核和涂阳肺结核检出率分别为0.01%和0.05%;有症状者肺结核和涂阳肺结核检出率分别为0.38%和0.93%,无症状者和有症状者肺结核和涂阳肺结核检出率均有统计学差异(P<0.05)。来源于密切接触者的患者与同期登记的来源于密切接触者之外的其他活动性肺结核患者之间,其性别、年龄构成和登记分类差异均有统计学意义(P<0.05)。结论应进一步加大涂阳肺结核密切接触者筛查力度,以便早期发现和治疗肺结核患者。

Objective To analyze the prevalence of close contacts of smear-positive PTB,and provide feasible suggestions for future screening of close contacts. Methods To collect the quarterly report of close contacts of smear positive PTB,annual report of patient registration and medical report information of patients detected from close contacts from 2009 to 2014 in Hubei. Data were analyzed by SPSS 11. 5. Results From 2009 to 2014,275 069 close contacts of smear-positive PTB were registered,and 271 427 were screened,among which 610 cases were diagnosed as active PTB,and 242 were smear-positive. The detection rate was 0. 49% and 0. 19%respectively. The detection rates of active PTB and smear positive in the asymptomatic were 0. 01% and 0. 05%,respectively. The detection rates of active PTB and smear positive in the symptomatic were 0. 38% and 0. 93%,respectively. There was difference of statistics in the rate of active PTB and smear positive between asymptomatic and symptomatic（ P 0. 05）. In addition,there was significant difference of gender,age and registration type between patients from close contacts and other non-close contacts registered at the same period（ P 0. 05）. Conclusion It should be strengthened to screen close contacts of smear-positive PTB for early detection and treatment of PTB.
