涎腺黏液表皮样癌CT的诊断

CT Diagnosis of Mucoepidermoid Carcinoma in Salivary Glands

目的探讨涎腺黏液表皮样癌的CT表现。方法回顾性分析经手术病理证实的27例涎腺黏液表皮样癌的CT表现,对肿瘤的部位、大小、形态、边缘、CT密度及强化方式进行分析。结果肿瘤位于腮腺12例,颌下腺3例,腭部7例,颊部3例,磨牙后三角区1例,上颌窦1例。低度恶性19例,中度恶性5例,高度恶性3例。病灶最大者4.8 cm,最小者0.7 cm;轮廓规则10例,不规则17例;边缘清晰9例,不清18例;18例增强扫描显著强化11例,中度强化6例,轻度强化1例;骨质破坏5例,淋巴结转移4例。结论 CT对涎腺黏液表皮样癌可以做出准确的定位、定量诊断,对肿瘤的定性诊断及分级有一定帮助。

Objective To explore the CT findings of mucoepidermoid carcinoma in salivary glands and enhancement characteristics of the disease. Methods The CT characteristics in 27 patients （ 12 males and 15 females, median age 43 years old） with pathologically proved mucoepidermoid carcinoma were retrospectively reviewed. CT were evaluated in rela- tion to the following： location, size, morphology, margin, CT density and enhancement pattern. Results Twelve lesions were located in parotid gland, 3 lesions in submandibular gland, 7 lesions in pars palatal gland, 3 lesions in buccal, 1 le- sion in retro molar triangle, and 1 lesion in maxillary sinus. Nineteen cases were low grade malignant tumors, 5 cases were moderate malignant tumors, and 3 cases were highly malignant tumors. The size of lesions ranged from 0.7 - 4.8 cm. The margins in 9 lesions were well defined, and 18 lesions were poorly defined. The contours of 10 lesions were regular, and 17 were irregular. On post contrast images,lesions demonstrated remarkable enhancement in 11 cases, moderate enhancement in 6 cases and mild enhancement in 1 case. Bone destruction in 5 cases and lymph node metastasis in 4 cases were noted. Conclusion CT can provide accurate evidence for localization and quantitation diagnosis, and it was helpful for histologi- cal diagnosis and to grading mucoepidermoid carcinoma in salivary glands.
