BiPAP呼吸机治疗COPD急性加重期合并急性左心衰患者的疗效观察

Effect observation of Bi PAP ventilator on the treatment of acute exacerbation of COPD patients with acute left heart failure

目的探讨Bi PAP呼吸机治疗COPD急性加重期合并急性左心衰的临床疗效。方法将56例COPD急性加重期合并急性左心衰患者随机分为观察组和对照组。对照组给予常规药物治疗,观察组则在常规药物治疗基础上加用Bi PAP呼吸机无创通气。检测2组患者治疗前后的动脉血气变化、血氧饱和度以及心率、血压等指标。结果观察组治疗总有效率为92.8%,显著高于对照组的78.5%(P<0.05);2组患者治疗7 d后RR、HR、SBP指标均较治疗前显著改善,且观察组的改善效果更为明显(P<0.05);2组患者治疗7 d后p H值、p(O2)、p(CO2)、BNP均较治疗前明显改善,且观察组的改善效果较对照组更为显著(P<0.05)。结论 Bi PAP无创呼吸机治疗疗效显著,可缩短住院时间和节省医疗费用,值得推广应用。

Objective To investigate the clinical effect of Bi PAP ventilator on the treatment of acute exacerbation of COPD patients with acute left heart failure. Methods A total of 56 acute exacerbation of COPD patients with acute left heart failure were randomly divided into observation group and control group. The control group was treated with conventional drug treatment,while the observation group was treated with Bi PAP ventilator and conventional drug treatment. The change of arterial blood gas,blood oxygen saturation,heart rate and blood pressure were measured before and after treatment in two groups. Results The total effective rate was 92. 8% in the observation group,which was significantly higher than 78. 5% in the control group（ P 0. 05）. RR,SBP and HR 7 days after treatment in two groups significantly improved,and these indexes in the observation group were significantly better than those in the control group（ P 0. 05）. The p H,p（ O2）,p（ CO2） and BNP 7 days after treatment in two groups significantly improved,and these indexes in the observation group were significantly better than those in the control group（ P 0. 05）. Conclusion Bi PAP ventilator is an effective method for treating acute exacerbation of COPD,which can shorten the hospital stay and save medical cost,so it is worthy of application and popularization.
