炎性因子、皮质醇、5-羟色胺与抑郁严重程度关系的初步研究

A preliminary study about associations of inflammatory factors,cortisol,5-HT and depression severity

目的探讨炎性因子(IL-6、TNF-a)、皮质醇及5-羟色胺(5-HT)与抑郁症状严重程度的关系。方法选取2014年5月至2015年5月在我院治疗的96例抑郁症患者为观察组,另选取40例健康体检者为对照组,采用ELISA法测定两组IL-6、TNF-a、皮质醇及5-HT水平。采用Pearson相关分析HAMD评分与炎性因子、皮质醇、5-羟色胺的关系。结果观察组血清IL-6、TNF-a、皮质醇显著高于对照组,而5-HT水平则低于对照组,差异有统计学意义(P<0.05)。与轻度、中度抑郁组相比,重度抑郁组血清IL-6、TNF-a、皮质醇水平较高,而5-HT水平则显著下降(P<0.05)。经Pearson相关分析,IL-6、TNF-a、皮质醇与HAMD评分呈正相关(r=0.315-0.389,P<0.05);5-HT与HAMD评分呈负相关(r=-0.326,P<0.05)。结论 IL-6、TNF-a、皮质醇及5-HT水平与抑郁严重程度有密切的关系,通过测定IL-6、TNF-a、皮质醇及5-HT水平可对抑郁症患者病情发展及预后进行评价。

Objective To explore the relationship among inflammatory factors（ IL 6,a TNF）,cortisol,5-HT and the severity of depressive symptoms. Methods 96 cases with depression cured in our hospital from May2014 to May 2015 were selected as observation group,another 40 cases of healthy physical examination were selected as control group. The level of IL-6,TNF-a,cortisol and 5-HT of two groups were measured by the method of ELISA. The relationship among HAMD score and inflammatory factors,cortisol,5-HT were evaluated by Pearson correlation analysis. Results The serum levels of IL- 6,TNF- a,cortisol in observation group was significantly higher than that in control group（ P〈0. 05）,and the 5-HT level in observation group was significantly lower than that of the control group（ P〈0. 05）. Compared with mild and moderate patients,the serum levels of IL-6,TNFa,cortisol levels of severe depression patients were significantly higher（ P〈0. 05）,while as the 5-HT levels of severe depression patients was significantly lower（ P〈0. 05）. The results of Pearson correlation analysis show that IL-6,TNF- related and cortisol were positive correlated with HAMD score（ P〈0. 05） and 5-HT was negative correlated with HAMD score（ P〈0. 05）. Conclusion Level of IL- 6 and TNF-a,cortisol and 5-HT has close relationship with the severity of depression. We may be able to evaluate the degree and prognosis of depression by detect the levels of IL-6,TNF-a,cortisol and 5-HT.
