MR及CT对腹膜后肿瘤诊断的临床价值分析

目的探讨腹膜后肿瘤的临床特征及MR,CT的影像学表现。方法选取2011年1月-2014年12月咸宁市某医院收治的58例腹膜后肿瘤患者作为研究对象,分析腹膜后肿瘤影像学特征,并以病理检测结果为金标准评估MR及CT 2种影像学检查方法的敏感性、特异性及准确性。结果腹膜后肿瘤患者以年龄超过49岁的中老年人居多,占81.0%;其中良性病变23例,占39.7%;恶性病变35例,占60.3%。病变类型以脂肪肉瘤、神经鞘瘤及副神经节瘤最常见,分别占25.9%,24.1%和20.7%。MR正确诊断恶性病变34例、良性病变23例,其中假良性1例,假恶性0例,准确性为98.3%(57/58),特异性为100.0%(23/23),敏感性为97.1%(34/35);CT正确诊断恶性病变32例和良性病变23例,其中假良性3例,假恶性0例,准确性为94.8%(55/58),特异性为100.0%(23/23),敏感性为91.4%(32/35)。结论 MR及CT联合检查可有效提高腹膜后肿瘤早期诊断的有效性,对准确判断肿瘤的性质具有积极意义,值得临床推广。

Objective To discuss the clinical presentation of retroperitoneal tumor and its MR and CT appear- ance. Methods The study ：onducted by selecting 58 cases of patients with retroperitoneal tumor of one hospi- tal of Xianning district of Hubei from January 2011 to September 2014. Imaging and clinical feature of retro- peritoneal tumor were analyzed. Methods of MR and CT, based on results of pathological examination, were compared in terms of sensibility, specificity and accuracy. Results Patients with retroperitoneal tumor aged o- ver 49 years had the highest incidence （81.0%）, among which 23 cases were benign lesions （39.7%） ; 35 ca ses were malignant lesions （60.3 %）. The lesion types focused on liposarcoma, schwannoma and paraganglio- ma, occupying 25.9%, 24.1% and 20.7% respectively. Of the 34 cases correct diagnosis of malignant lesion and of 23 cases correct diagnosis of benign lesion by employing MR, 1 of 23 was false benign lesion and 0 of 34 was false malignant lesion, manifesting 98.3~ accuracy （57/58）, 100.0% specificity （23/23）and 97.1% sen- sibility （34/35）. Of the 32 cases correct diagnosis of malignant lesion and of 23 cases correct diagnosis of be- nign lesion by employing CT, 3 of 23 were false benign lesion and 0 of 32 was false malignant lesion, showing 94.8% accuracy （55/58）, 100.0% specificity （23/23） and 91.4%sensibility （32/35）. Conclusion Combina- tion use of MR and CT can improve early diagnosis efficiency and diagnosis accuracy in patients with retroper- itoneal tumor, which is worthy to be popularized.
