摘要
目的评价癌胚抗原(CEA)、糖链抗原153(CA153)和C反应蛋白(CRP)单独检测和联合检测用于评价乳腺癌的诊断价值。方法分别检测CEA、CA153和CRP在正常对照组、乳腺增生和乳腺癌的含量。二元Logistic回归分析用于评价其联合诊断价值。结果与正常对照组相比较,乳腺增生患者CA153和CRP的含量显著升高,CEA差异无统计学意义,乳腺癌患者CEA、CA153和CRP的含量均显著升高。与乳腺增生患者相比较,乳腺癌患者CEA、CA153和CRP的含量均显著升高。当CEA、CA153和CRP单独检测时,CRP的诊断价值最高,其曲线下面积为0.82,其诊断灵敏性和特异性分别为78.43%和76.86%。联合检测区分乳腺增生和乳腺癌的曲线下面积为0.95,其灵敏性和特异性分别为89.53%和98.41%,诊断方程为Y=logit(P)=1.448-0.247 XCEA-0.075 XCA153-0.741 XCRP。与单独检测相比,二元Logistic回归分析ROC曲线下面积具有统计学差异(P<0.05)。结论 CEA、CA153和CRP联合检测提高乳腺癌的诊断价值,可以作为潜在的临床乳腺癌的辅助诊断方法。
Objective To evaluate the diagnostic value of alone and combined detection of carcinoembryonic antigen(CEA), carbohydrate antigen 153(CA153), and C reactive protein(CRP) in breast cancer. Methods The serum concentration of CEA, CA153, and CRP were detected separately in the healthy control group, the mammary gland hyperplasia group and breast cancer group. Binary logistic regression analysis was used to evaluate the joint diagnostic value. Results Compared with the healthy control group, the serum levels of CA153 and CRP in the mammary gland hyperplasia group were significantly increased, but no significant difference on CEA levels. The serum levels of CEA, CA153 and CRP in the breast cancer group were significantly higher than that of mammary gland hyperplasia group and healthy controls. When the serum CEA, CA153 and CRP levels were used alone for differentiating the mammary gland hyperplasia group and breast group, the CRP had the best diagnostic value. The area under curve was 0.82. The diagnostic sensitivity and specificity were 78.43% and 76.86% respectively. When combined the serum CEA, CA153 and CRP detection, the area under curve was 0.95, the sensitivity and specificity were 89.53% and 98.41% respectively. Conclusion The combined detection of CEA, CA153 and CRP could improve the diagnostic value of breast cancer, and may be used as a potential diagnostic method for breast cancer.
