HPV16/18感染对宫颈病变早期筛查的价值

THE VALUE OF HPV16/18 POSITIVE IN EARLY SEREENING OF CERVICAL LESIONS

目的通过高危型HPV16/18感染状况分析,探讨HPV感染对宫颈病变早期筛查的价值。方法选择227例不同宫颈病变患者作为研究对象,以妇科健康体检者138例作为对照。应用聚合酶链反应(PCR)分析宫颈脱落细胞HPV16/18感染状况。结果宫颈炎患者93例,HPV16阳性6例,HPV18阳性4例,二者均阳性1例;宫颈上皮细胞内瘤变(包括CINⅠ、Ⅱ及Ⅲ级)患者98例,HPV16阳性24例,HPV18阳性10例,二者均阳性7例;宫颈癌患者36例,HPV16阳性15例,HPV18阳性5例,二者均阳性9例。宫颈上皮细胞内瘤变与宫颈癌患者中,HPV16感染较HPV18感染多见,HPV16/18阳性率随宫颈病变的严重程度增加而上升,差异有统计学意义(P<0.01)。三组不同宫颈病变患者的HPV16/18感染的年龄分布也存在差异,差异有统计学意义(P<0.01)。结论 HPV16和18感染阳性率与宫颈病变的严重程度密切相关,提示可通过HPV16和18感染的早期筛查,及时发现宫颈病变患者。

Objective To explore the value of HPV infections in early screening of cervical lesions through analyzing high-risk HPV type 16/18 infection.Methods HPV16/18 infection in cervical exfoliated cells of 227 different cervical lesions patients were analyzed by polymerase chain reaction（PCR）,and to use 138 cases physical examination of gynaecology as control.Results From93 cervicitis samples,6 samples were HPV16 positive,4 samples were HPV18 positive,and 1 sample was mixture for HPV16 and 18 positive;from98 cervical intraepithelial neoplasia（including CIN Ⅰ,Ⅱ and Ⅲ）patients,24 cases were HPV16 positive,10 cases were HPV18 positive,7 cases were mixture for HPV16 and 18 positive;From36 cervical cancer patients,15 cases were HPV16 positive,5 cases were HPV18 positive,9 cases were mixture for HPV16 and 18 positive.HPV16 infection is more common than HPV18 infection in cervical intraepithelial neoplasia and cervical cancer patients,and HPV16/18 positive rate increased with the severity of cervical lesions,the difference was statistically significant（P〈0.01）.The age distribution of HPV16/18 infection in three different cervical lesions groups were also different,and statistical analysis showed that they are statistically difference（P〈0.01）.Conclusion HPV16 and 18 positive rate is closely related to the severity of cervical lesions that suggests the cervical lesion patients can be diagnosed in time by early screening HPV16/18 infection.
