放疗联合鸦胆子油乳治疗小细胞肺癌脑转移的疗效观察

CLINICAL RESEARCH OF STEREOTACTIC RADIOTHERAPY COMBINED WITH BRUCEA JAVANICA OIL ON BRAIN METASTASIS OF SMALL-CELL LUNG CANCER

目的探讨鸦胆子油乳联合放疗治疗小细胞肺癌脑转移的疗效和不良反应。方法分析该科鸦胆子油乳联合放疗70例(观察组)和羟喜树碱(HCPT)联合放疗(对照组)70例,全脑照射基础剂量为30Gy,残留病灶≤3个并且残留病灶>1cm时,缩野对局部病灶追加剂量20 Gy;残留病灶>3个或残留病灶≤1cm时,继续全脑追加剂量10Gy,对照组在放疗期间给予HCPT6mg/m2,观察组:鸦胆子油乳注射液30ml/d,放疗第一天开始至放疗结束。结果总有效率观察组77.14%,对照组80.00%,二组有效率比较差异无统计学意义(P>0.05)。二组1、2年生存率差异无统计学意义。不良反应二组比较差异有统计学意义(P>0.05)。结论放疗联合鸦胆子油乳治疗小细胞肺癌脑转移疗效基本等同于放疗联合HCPT,是一种安全经济、高效的治疗手段。

Objective To explore the efficacy and side reaction in patients on brain metastasis of small-cell lung cancer of stereotactic radiotherapy combined with Brucea javanica oil.Methods A retrospective analysis was javanica oil emulsion combined with radiotherapy 70 cases（study group）and hydroxycamptothecin（HCPT）combined with radiotherapy（control group）70 cases,the basis of whole brain irradiation dose was 30 Gy,the total number of lessions was less than or equal three,and the diameter of each lesion was greater than 1cm involved field was added dose 20 Gy;the total number of lessions was greater than three,and the diameter of each lesion was less than or equal 1cm,whole brain was added dose 10 Gy.the control group was given HCPT6 mg/m2 d1-5,observation group：javanica oil emulsion injection30 ml every day during radiotherapy,the results of the total efficiency：observation group was 77.14%,control group was80.00%,there were no significant difference（P〉0.05）.Two 2-year survival rate was not significant different.there was a significant difference of side reaction of the two groups（P〉0.05）.Conclusion Radiotherapy combined with brucea javanica oil is a efficacious methods for the treat Brain Metastasis of smallcell lung cancer.It is a safe,economic,efficient method of treatment.
