摘要
目的应用实时三维超声心动图(RT-3DE)评价糖耐量异常患者左室功能及心肌质量的情况。方法选取我院门诊及住院糖耐量异常患者70例(糖耐量异常组)和健康对照组70例,应用RT-3DE检测两组左室心肌质量指数(LVMI)、左室舒张末容积指数(LVEDVI)、左室每搏指数(LVSVI)及左室射血分数(LVEF)。同时组织多普勒成像检测左室舒张功能参数Ea、Aa及Ea/Aa,比较两组上述各指标。结果糖耐量异常组Aa较健康对照组偏大(P<0.05),Ea、Ea/Aa较健康对照偏小(均P<0.05),而两组LVEDVI、LVSVI、LVEF无明显差异,但糖耐量异常组的LVMI较健康对照组偏高(P<0.05),且与患者的血糖水平呈正相关(r=0.557,P<0.05)。结论糖耐量异常可以导致患者左室心肌重塑,影响患者的左室舒张功能。
Objective To investigate the function and myocardia mass of left ventricle in patients with abnormal glucose tolerance by real-time three-dimensional ultrasound(RT-3DE).Methods Seventy cases with abnormal glucose tolerance and 70 age and sex-matched healthy controls were involved in this study.RT-3DE was used to measure LVMI,LVEDVI,LVSVI and LVEF.Meanwhile,tissue Doppler imaging was used to measure Ea,Aa and Ea/Aa.Results Aa was increased in patients with abnormal glucose tolerance compared to controls(P0.05),while Ea and Ea/Aa were decreased in patients with abnormal glucose tolerance when compared to healthy controls(all P0.05).There were no difference in LVEDVI,LVSVI and LVEF between two groups.In addition,LVMI of patients with abnormal glucose tolerance was increased when compared to the controls(P0.05).There was a positive correlation between LVMI and glucose levels(r=0.557,P0.05).Conclusion Abnormal glucose tolerance can induce myocardial remodeling and affect the left ventricular diastolic function.
