超声联合磁共振胆胰管成像在诊断胆总管结石中的应用价值

Diagnostic value of choledocholithiasis by ultrasound combined with MRCP

目的探讨超声联合磁共振胆胰管成像(MRCP)对胆总管结石的诊断价值。方法选取疑诊为胆总管结石的患者135例,均行超声、MRCP检查,以术中探测或经内镜逆行性胰胆管造影术取出结石为金标准,比较超声联合MRCP与单纯超声、单纯MRCP对胆总管结石的诊断价值。结果 135例患者经手术或ERCP确诊胆总管结石109例,二者联合诊断胆总管结石的敏感性、特异性和准确率均较单独超声诊断高,差异均有统计学意义(P<0.05);二者联合诊断胆总管结石的准确率较单一MRCP诊断高,差异有统计学意义(P<0.05),其余指标无差异。结论超声联合MRCP检查可提高胆总管结石的诊断准确率,值得临床应用。

Objective To evaluate the diagnostic value of ultrasound combined with MRCP in choledocholithiasis diagnosis.Methods A total of 135 patients with suspicious common bile duct calculus were enrolled.Data of 135 patients detected by both ultrasonography and MRCP were compared with the postoperative results.The diagnostic accuracy,sensitivity and specificity of choledocholithiasis using ultrasound combined with MRCP were calculated.Results Altogether 109 patients were postoperatively diagnosed as choledocholithiasis.Compared with respectively using ultrasound and using MRCP,the accuracy,sensitivity,specificity of using ultrasound combined with MRCP were higher than using single examination（P 0. 05）.Conclusion Using ultrasound combined with MRCP can improves the accuracy of diagnosis,thus,it is worth of clinical application.
