脂肪肝声像图分度在脂肪肝及相关内分泌代谢疾病诊断中的应用价值 被引量:8

Study on relationship between the sonogram degree of fatty liver and the related diseases of endocrine and metabolic system

摘要 目的探讨脂肪肝声像图分度在脂肪肝及相关内分泌代谢疾病诊断中临床应用价值。方法选取经超声诊断的脂肪肝患者562例,依据声像图分度将其分为轻度脂肪肝222例、中度脂肪肝246例及重度脂肪肝94例,比较分析不同声像图分度与相关实验室指标,以及是否合并冠状动脉粥样硬化性心脏病、高血压病、颈动脉硬化、下肢动脉硬化、高脂血症及糖尿病等代谢疾病的关系。结果轻、中、重度患者体质量指数、甘油三酯、谷草转氨酶及谷丙转氨酶比较差异有统计学意义(P<0.05);血清甘油三酯、谷草转氨酶及谷丙转氨酶与脂肪肝声像图分度呈正相关(r=0.66、0.64、0.52,P<0.01),体质量指数、总胆固醇、低密度脂蛋白、空腹血糖、收缩压及舒张压与脂肪肝声像图分度呈正相关(r=0.25、0.03、0.22、0.25、0.18、0.25,均P<0.05),高密度脂蛋白与脂肪肝声像图分度呈负相关(r=-0.15,P<0.01)。结论脂肪肝的声像图分度可反映脂肪肝及其相关代谢性疾病的发病趋势及严重程度。应用脂肪肝声像图分度对脂肪肝进行临床诊断与评估具有良好的应用价值。 Objective To investigate the clinical application value between fatty liver sonogram degree and the related diseases of endocrine and metabolic system.Method According to the ultrasonographic diagnosis,562 cases of fatty liver were enrolled in this study,and they were divided into three degrees,the mild fatty liver(222 cases),the moderate fatty liver(246 cases)and severe fatty liver(94 cases).Compared the sonogram degree of fatty liver and their corresponding laboratory tests in related endocrine and metabolic diseases,such as coronary heart disease,hypertension,carotid artery atherosclerosis,hyperlipidemia and diabetes mellitus.Result Over half of the fatty liver cases also suffered from obesity,hypertension,diabetes,hyperlipidemia and arteriosclerosis.The degree of fatty liver was significantly corresponding to the levels of BMI,TG,ALT and AST.Conclusion The sonogram degree is valuable to evaluate relationship between the fatty liver and the related diseases of the endocrine and the metabolic system.
