床旁颅脑B超对新生儿脑损伤的诊断价值 被引量:21

摘要 目的探讨床旁颅脑B超对新生儿脑损伤的诊断价值。方法选取我院新生儿病室具有脑损伤高危因素的572例患儿,应用扇形探头与高频线阵探头结合检查的方法进行床旁颅脑超声检查,并与CT检查及临床诊断进行对照分析。结果 572例患儿中超声检查阳性发现151例,包括新生儿颅内出血82例,新生儿缺氧缺血性脑损伤11例,新生儿缺氧缺血性脑损伤合并颅内出血18例,室管膜下囊肿40例;复查CT检查117例,阳性发现98例;临床诊断新生儿脑损伤共135例,其中缺氧缺血性脑病合并颅内出血67例,缺氧缺血性脑病15例,颅内出血53例。结论床旁颅脑B超检查无创且简便易行,应用扇形探头与高频探头相结合,对脑室周围-脑室内出血等有较高的诊断价值,对缺氧缺血性脑病、脑实质内点状出血、硬膜下出血及蛛网膜下腔出血敏感性不如CT,可作为新生儿脑损伤的首选影像筛查项目。 Objective To study the diagnostic value in neonatal brain injury using bedside B-mode uhrasonography. Methods Five hundred and seventy-two newborns with high risk of brain damage were enrolled. Data of bedside ultrasonography,CT scan results and clinical information were analyzed.Results In 572 patients, 151 cases were positive, including 82 cases of neonates intraeranial hemorrhage, 11 cases of hypoxicisehemie brain damage, 18 cases of hypoxicischemic brain damage combined with intracranial hemorrhage, 40 cases of sub-ependymal cysts. 117 cases were performed CT scan, 98 of them were positive.Altogether 135 cases were diagnosed as brain damage according to clinic materials,including 67 cases of hypoxicischemic brain damage combined with intracranial hemorrhage, 15 cases of hypoxicischemie brain damage ,53 cases of intracranial hemorrhage.Conclusion Bedside uhrasonography is noninvasive, convenient method for newborns to diagnose brain damage, and can be used to screen brain injury for newborns.
