长三角地区流动人口医疗保险政策研究

The Floating Population Health Insurance Policy Research of Yangtze River Delta Region

文章通过梳理2002~2014年长三角地区流动人口医疗保险政策,将区域政策演化脉络分为政策暗流期、政策开源期和政策汇流期,并从公共政策视角出发,着重运用多源流模型和政策绩效评价方法对该地区政策进行分析。研究认为,城镇化是社会发展的必然趋势,流动人口的医疗保险政策应以之为框架进行改革和设计。长期以来长三角地区围绕＂稳定＂解决＂流动＂的政策思路无论是在理论上还是实践中均有较大的局限性,对流动人口而言＂稳定＂这一参保门槛较高。政府可尝试为流动人口建立＂基本水平、广泛覆盖、多方负担、统账结合、可转可续、过渡衔接＂的医疗保险制度,将其作为未来流动人口融入城市医疗保险体系的过渡性医疗政策。

On the basis of listing the health insurance policy of the floating population of Yangtze River Del-ta region in 2002~2014,the regional policy evolution context can be divided into undercurrent period,wideopening period and combining period. This paper especially using Multiple origin model and Policy perfor-mance evaluation to analyze the regional policy from the perspective of public policy. The results suggestedthat the urbanization is the inevitable trend of social development,and the health insurance policy of float-ing population should be reformed and designed in the framework. Whatever in theory or practice,the blue-print focused on stable contract relationship to solve floating problems has big limitations,so it is unavail-able for the floating population to get into the health care system. Maybe there is a new medical insurancesystem as a transition for the floating population getting into urban that includes several principles：basiclevel,wide coverage,responsibility shared,social pooling-individual accounts,sustainable,and transitionalfunction.
