摘要 目的探讨男性急性心肌梗死(AMI)患者血清睾酮(T)及唾液酸(SA)水平的变化及临床意义。方法收集2014年6月武汉大学人民医院的男性急性心肌梗死患者93例(病例组)及体检健康男性74例(对照组),分别检测其血清T、雌二醇(E2)、SA的浓度,并对结果进行统计学分析。结果病例组患者血清T水平为(339.72±122.05)ng/dl,明显低于对照组的(387.63±145.71)ng/dl,差异有统计学意义(P<0.05),血清SA水平为(682.66±128.96)mg/L,显著高于对照组的(584.86±104.07)mg/L,差异有显著统计学意义(P<0.01);男性AMI患者血清SA与T及T/E2比值呈负相关(r分别为-0.211、-0.162,P均<0.01);血清T与E2呈正相关(r=0.363,P<0.01);用血清SA诊断AMI时,曲线下面积为75.2%(P<0.01),临界值为599 mg/L,此时灵敏度为75.3%,特异度为66.2%。结论检测男性AMI患者血清T及SA对其疾病的预防、诊断、治疗等都具有重要意义。 Objective To investigate the changes and clinical significance of serum testosterone and sialic acid in male patients with acute myocardial infarction. Methods Ninety-three male patients with acute myocardial infarction(the study group) and 74 healthy males(the control group) who visited Renmin Hospital of Wuhan University in June, 2014 were collected. The levels of serum testosterone(T), estradiol(E2) and sialic acid(SA) were detected,and then compared between the two groups. Results The serum T level was(339.72 ± 122.05) ng/dl in the study group, significantly lower than(387.63±122.05) ng/dl in the control group(P〈0.05). The serum SA level in the study group [(682.66 ± 128.96) mg/L] was significantly higher than that in the control group [(584.86 ± 104.07) mg/L], P0.001. There was a negative correlation between serum SA and T(r=-0.211, P〈0.01), SA and T/E2(r=-0.162, P〈0.01,and a positive correlation between T and E2(r=0.363, P〈0.01) in the male patients with AMI. Using serum SA to diagnose the AMI, the area under the curve was 75.2%(P〈0.01), with the critical value of 599 mg/L, sensitivity of 75.3%,and specificity of 66.2%. Conclusion Combined detection of serum T and SA has great significance in the prevention, diagnosis and treatment of male patients with AMI.
