当归四逆汤治疗肩关节周围炎50例临床观察

Clinical observation of Danggui- sini decoction on the treatment of scapulohumeral periarthritis

目的观察当归四逆汤治疗肩关节周围炎(以下简称肩周炎)的临床疗效。方法将150例肩周炎患者随机分为3组,当归四逆汤组50例予当归四逆汤治疗,葛根汤组50例予葛根汤治疗,双氯芬酸二乙胺乳胶剂组50例予双氯芬酸二乙胺乳胶剂治疗,并辅以适当推拿治疗,3组均10 d为1个疗程,治疗3个疗程后比较3组疼痛疗效,观察3组视觉模拟评分法(VAS)评分、肩关节功能评分变化。结果当归四逆汤组总有效率高于葛根汤组、双氯芬酸二乙胺乳胶剂组(P<0.05),当归四逆汤组疗效优于葛根汤组、双氯芬酸二乙胺乳胶剂组。葛根汤组与双氯芬酸二乙胺乳胶剂组总有效率比较差异无统计学意义(P>0.05),2组疗效相当。治疗后当归四逆汤组VAS评分低于葛根汤组、双氯芬酸二乙胺乳胶剂组(P<0.05),葛根汤组与双氯芬酸二乙胺乳胶剂组VAS评分比较差异无统计学意义(P>0.05)。3组治疗后肩关节功能评分均较本组治疗前升高(P<0.05),且当归四逆汤组肩关节功能评分中的肩关节内旋、摸背、摸耳和总评分升高更明显,优于其他2组(P<0.05)。葛根汤组与双氯芬酸二乙胺乳胶剂组肩关节功能评分比较差异无统计学意义(P>0.05),2组肩关节功能改善相当。结论当归四逆汤治疗肩周炎,疗效突出,为治疗肩周炎较为理想方药。

Objective To observe the clinical effect of Danggui - sini decoction on scapulohumeral periarthritis. Methods 150 patients with scapulohumeral periarthritis were randomly divided into three groups. 50 patients in Danggui - sini decoction group were treated by Danggui - sini decoction. 50 patients in Gegen decoction group were treated by Gegen decoction. 50 patients in diclofenac diethylamine emulgel group were treated by dielofenac diethylamine emulgel and appropriate massage therapy. The course was 10 days in three groups. The pain effect after three courses was compared among three groups. The score changes of visual analogue scale （ VAS） and shoulder joint were observed in three groups. Results The total effective rate in Danggui - sini decoction group was higher than that in Gegen decoction group and diclofenac diethylamine emulgel group （P 〈 0.05 ）, the curative effect in Danggui -sini decoction group was superior to that in Gegen decoction group and diclofenac diethylamine emulgel group. There was no significant difference between Gegen decoction group and diclofenac diethylamine emulgel group on total effective rate （P 〉 0.05 ）, and the curative effect was similar. The VAS score in Danggui - sini decoction group was lower than that in Gegen decoction group and diclofenac diethylamine emulgel group （ P 〈 0.05 ）. There was no significant difference between Gegen decoction group and diclofenac diethylamine emulgel group on VASscore （ P 〉 0.05 ） ： The shoulder joint score after .treatn4ent was increased in three groups （ P 〈 0.05 ）. The scores of shoul-der internal rotator, touch the back, touch the ear and total score after treatment in Danggni - sini decoction group were significantly increased, and the increase was obvious superior to that in other two groups （P 〈 0.05 ） There was no significant difference between Gegen decoction group and diclofenac diethylamine emulgel group on the shoulder joint score （P 〉 0. 05）, and the shoulder joint function was similar. Conclusion Danggui - sini decoction has remarkable curative effect on the treatment of scapulohumeral periarthritis which is the ideal medicine for the treatment of scapulohnmeral periarthritis.
