不同针刺时机、选穴配伍搭配方案对脑梗死肌张力异常患者简化Fugl-Meyer运动功能的影响

Effects of different acupuncture opportune moment combined with acupoint compatibility on simplify Fugl-Meyer assessment scale in cerebral infarction patients with dystonia

目的 观察不同针刺时机、选穴配伍搭配方案对脑梗死肌张力异常患者简化Fugl-Meyer运动功能的影响。方法 以脑梗死肌张力异常患者90例为观察对象,在内科、康复等标准规范治疗基础上,采用正交试验设计法,对针刺时机（病程1-10 d、11-20 d、20-30 d）、选穴配伍（头穴＋阴经穴、头穴＋阳经穴、头穴＋阴经与阳经穴交替）、电针使用（不加电针、电针断续波、电针疏密波）、疗程（2周、4周、6周）之四因素三水平不同搭配组合方案,对脑梗死肌张力异常患者进行简化Fugl-Meyer运动功能评定,初步确定最佳搭配方案,为临床应用针刺治疗脑梗死肌张力异常提供指导方案。结果 脑梗死病程1-10 d、选择头穴＋阴经与阳经穴交替针刺、电针断续波、疗程6周对简化Fugl-Meyer运动功能起相对最佳改善作用。

Objective To observe the effects of different acupuncture opportune moment combined with acupoint compatibility on simplify Fugl - Meyer assessment scale in cerebral infarction patients with dystonia. Methods 90 cerebral infarction patients with dystonia were selected. On the basis of the internal medicine and rehabilitation standard treatment, .the combined scheme of acupuncture opportune moment （1 - 10 d, 11 -20 d, 20 - 30 d）, acupoint compatibility （scalp points ＋ yin meridian points, scalp points ＋ yang meridian points, scalp points ＋ alternately yin and yang meridian points）, electroacupuncture application （without electroacupuncture, interrupted wave, dilatational wave） and therapeutic course （two weeks, four weeks, six weeks） were optimized by orthogonal design of four factors and three levels. The simplify Fugl - Meyer motor function in cerebral infarction patients was evaluated and the best arrangement scheme was initially determined for the guidance of the acupuncture therapy on cerebral infarction. Results The timing of cerebral acupuncture course within 1 - 10 days, electroacupuncture the points of scalp points ＋ yin and yang meridian points alternately in interrupted wave for 6 weeks has the best improvement effect on simplify Fugl - Meyer motor function.
