1例肾移植术后股骨头坏死患者的药学监护

Pharmaceutical care on a patient with femoral head necrosis after renal transplantation

1例43岁女性患者,因肾移植术后5个月,双胯部疼痛不适入院。外院查双髋关节MR示:双侧股骨头坏死(右侧较明显),双髋关节退行性变,右髋关节少量积液。入院后给予抗排斥、抗凝等治疗,同时给予中药熏洗、针灸结合热疗等处理。治疗期间,临床药师加强对患者的药学监护,主要包括:免疫抑制剂的使用、监测他汀类与免疫抑制剂的相互作用等,同时对患者服药注意事项、定期复查项目等进行用药教育,确保用药安全、有效,治疗8 d后患者病情好转出院。

One 43-year-old female patient with renal transplantation for 5 months was hospitalized for pain in crotch. The MR for bilateral hip joints showed bilateral femoral head necrosis, hip joint retrogression and few hydrops in right hip joint. After hospitalization, the patient received the antirejection and anticoagulant therapy. At the same time, herbal fumigation and washing, acupuncture combined with thermotherapy were used. During the treatment, clinical pharmacists took pharmaceutical cares for making sure the safety and effi ciency of medication as follows： use of immunosuppressants, monitoring the adverse drug reaction and drug interaction of statins and immunosuppressants, and carrying out the medication education on cautions of drugs and regular reexamination indexes. After 8 days, the patient was in stable situation and discharged from hospital.
