肿瘤患者血型不规则抗体的分析

Analysis of blood irregular antibodies in cancer patients

目的:分析肿瘤患者人群中血型不规则抗体的检出频率及分布特点,为肿瘤患者的输血提供帮助。方法:对2013-01-2013-12住院的14 400例肿瘤患者应用盐水、抗人球蛋白法、聚凝胺法检测血型不规则抗体,同时鉴定出其特异性。结果:14 400例肿瘤患者中,不规则抗体阳性62例(0.43%),其中恶性肿瘤患者阳性率为0.56%(44/7854),与良性肿瘤的0.27%(18/6 546)比较,差异有统计学意义(P<0.05);女性阳性率0.65%(48/7398)高于男性0.20%(14/6 940),有输血史0.54%(42/7 750)高于无输血史30.0%(20/6 588),有妊娠1.10%(38/3 445)高于无妊娠0.25%(10/3 953),各组间比较差异均有统计学意义(均P<0.01);血型不规则抗体特异性中抗-E抗体居多,在各种肿瘤中的分布不均(P<0.05)。结论:恶性肿瘤患者不规则抗体阳性率高于良性肿瘤患者,女性患者不规则抗体阳性率与妊娠有关,输血前检测不规则抗体有利于保障临床用血,降低溶血性输血反应的发生。

Objective：To detect the frequency and distribution of blood irregular antibodies in cancer patient,and provide help for blood transfusion of cancer patients.Method：Blood irregular antibodies of 14,400 cases of cancer patients from January 2013 to December 2013 were detected by saline method,anti-human globulin method and polybrene test,and its specificity was identified.Result：Among 14 400 cases,62 cases（0.43%） was irregular antibody positive,including cancer patients positive rate 0.56%（44/7 854）and patients with benign tumors positive rate 0.27%（18/6 546）,which the difference was significant（P〈0.05）.The positive rate in female patients0.65%（48/7 398） was higher than that in male patients 0.20%（14/6 940）.The positive rate in patients with blood transfusion history 0.54%（42/7 750） was higher than that in patients without blood transfusion history30.0%（20/6 588）.The positive rate in pregnancy patients 1.10%（38/3 445） was higher than that in non pregnancy patients 0.25%（10/3 953）.All P values were less than 0.01.Anti-E antibody was the mostly antibody.Conclusion：Blood irregular antibody positive rate was higher in cancer patients than in patients with benign tumors,and the blood irregular antibody positive rate in women was related to pregnancy.Irregular antibody detection before blood transfusion could help for clinical blood use and reduce the incidence of hemolytic transfusion reactions.
