基于社区的2型糖尿病伴发抑郁现况及生命质量的研究

Community Based Study of Depression Prevalence and Quality of Life in T2DM Patients

目的了解2型糖尿病(T2DM)合并抑郁的患病情况及生命质量。方法采用分层随机抽样的方法调查成都市社区卫生中心随访的T2DM患者704人,采用自编调查问卷及抑郁自评量表(self-rating depression scale,SDS)进行调查。问卷使用Epidata 3.0录入数据库,采用SPSS 17.0统计软件进行统计分析。结果 T2DM患者抑郁患病率43.32%,轻、中、重度抑郁患病率分别为19.74%、19.18%和4.40%,男女差异无统计学意义(2=6.335,P=0.096)。T2DM伴抑郁患者SF-36生命质量量表中各维度得分均低于未伴抑郁者,差异有统计学意义(P<0.001)。结论 T2DM患者伴发抑郁患病率高,抑郁可导致T2DM患者生命质量变差。保持乐观积极的心态、加强锻炼、控制体质量、经常监测血糖、减少并发症的发生,有助于改善糖尿病患者的生命质量,促进患者康复。

Objective To investigate the prevalence of depression and the quality of life in patients with Type2diabetes（T2DM）.Methods 704 cases with type 2diabetes were selected by multi-stage random sampling in Community Health Service Centre in Chengdu.T2 DM patients were investigated by self-made questionnaire and self-rating depression scale（SDS）.Epidata 3.0database were used in designing questionnaire and SPSS 17.0was used for statistical analysis.Results The prevalence of depression in T2 DM group was 43.32%,among which mild depression was 19.74%,moderate depression was 19.18% and major depression was 4.40%.There was no statistically significant difference between men and women（2=6.335,P=0.096）.T2 DM patients with depression had lower quality of life as shown in SF-36,the difference of which was statistically significant（P 0.001）.Conclusion The prevalence of depression in T2 DM patients is high.Depression can cause the decline of quality of life in patients with diabetes mellitus.Maintaining apositive and optimistic attitude,regular exercise,weight control and regular monitoring of blood sugar can improve the quality of life and promote rehabilitation of patients with diabetes.
