摘要
目的探讨出院准备服务在老年2型糖尿病患者中的应用价值。方法累计收集80例老年2型糖尿病患者,随机分为接受常规护理的对照组和常规护理基础上进行出院准备服务的试验组,每组40例,比较2组患者住院期间和出院后3、6月相关临床数据的差异。结果与对照组比较,试验组患者出院后3、6月的糖化血红蛋白明显降低,出院后门诊复诊率增高,且用药更为规律,血糖自我监测率升高,急性并发症发生率和因急性并发症急诊入院率明显降低。结论在老年糖尿病患者管理中实施个案管理模式的出院准备服务可有效改善患者治疗依从性,使患者得到持续性照护,病情控制平稳,提高生活质量。
Objective To evaluate the effectiveness of discharge planning in the elderly patients with type 2diabetes. Methods Eighty elderly patients with type 2 diabetes were enrolled. The observation group with 40 patients received discharge planning service on basis of conventional nursing,and the control group with 40 patients received conventional nursing service. Results Three or six months after treatment,compared with control group,the level of HbA 1c was significantly lower,the referral rate of outpatient and the rate of blood glucose monitor were significantly higher,and the incidence rate of acute complications and emergency treatment was significantly lower in observation group.Conclusions Applying discharge planning in elderly patients with type 2 diabetes can improve the compliance of treatment,so that the patients can get continuing nursing,getting well disease control and higher quality of life.
